临床药师参与会诊72例抗菌治疗病例的分析

Clinical Pharmacist Participating in Consultation and Analysis of 72 Cases of Antibacterial Therapy

目的:了解临床药师参与会诊的实际情况和结果 ,为临床药师更好地参与临床药物治疗实践和临床药师职业发展提供参考,提升临床合理用药水平。方法:对2013年1月-2014年8月临床药师参与会诊的72例抗菌治疗会诊资料进行统计分析。结果:会诊原因有两种:制定抗感染治疗预案12例(16.7%);要求调整原治疗方案60例(83.3%)。会诊建议被临床全部采纳者65例(90.3%),部分采纳者5例(6.9%),未采纳者2例(2.8%)。意见采纳后治疗:痊愈56例(80%),好转11例(15.7%),效果不明显2例(2.9%),死亡或放弃治疗1例(1.4%)。结论:临床药师会诊已成为临床治疗的重要环节,尤其对抗菌药物的合理应用起到很大的作用;临床药师应积极参与会诊,强化临床药学思维,积累经验,拓展会诊范围,提高会诊质量,促进合理用药。

Objective： To understand the actual situation and results of clinical pharmacist ’ s participating in consultations so as to provide a reference for the better practice by clinical pharmacists in clinical drug treatment and their occupation development , and to improve the level of rational drug application . Methods： From January 2013 to August 2014 clinical pharmacists participated in the consultations of 72 cases of anti-infection treatment and the consultation data was analyzed statistically . Results：The purpose of consultation was to make preplans for the anti-infection treatment of 12 cases （16.7%） and to adjust the original treatment for 60 cases （83.3%）.About 90.3%of consultation suggestions were totally accepted , 6 . 9% were partially taken and 2 . 8% were not adopted . As the results , 56 cases were cured （ 80%） , the conditions of 11 cases improved （ 15 . 7%） , 2 cases （ 2 . 9%） without ob-vious improvement , and 1 case （ 1 . 4%） died or giving up treatment . Conclusion：Clinical pharmacist participating in consultation has already become an important part of clinical therapy , which has played a great role in rational use of antibacterials particularly . Pharmacist should participate actively in consultation , get used to intensive considera-tion of clinical pharmacy , accumulate experiences , expand the scope of consultation and improve the consultation quality so as to promote rational drug use .
