锁定钢板治疗复杂胫骨平台骨折的疗效分析

Curative effect analysis of locking plate for the treatment of complex tibial plateau fractures

目的探讨锁定钢板治疗复杂胫骨平台骨折的临床疗效。方法回顾性分析2011年1月-2014年1月在我院骨科应用锁定钢板内固定治疗的复杂胫骨平台骨折患者40例的临床资料,观察患者术后膝关节功能恢复的优良率及手术时间、术中出血量、骨折愈合时间、住院时间、并发症情况。结果 40例复杂胫骨平台骨折患者均顺利完成手术,术后膝关节功能恢复优良率92.5%,其中优20例、良17例、可2例,差1例。其手术时间（113.1±23.8）min、术中出血量（112.85±21.90）m L、术后住院时间（23.43±3.28）d、骨折愈合时间（13.52±2.14）周。术后无一例出现局部皮肤坏死、螺钉松动,其并发症发生率合计10%。结论锁定钢板内固定治疗复杂胫骨平台骨折能明显改善患者的膝关节功能,术后骨折愈合快、并发症少,值得临床推广和应用。

Objective To investigate the clinical effect of locking plate for treatment of complex tibial plateau fractures. Methods The clinical data of 40 cases of patients with complex tibial plateau fractures who treated with locking plate internal fixation from 2011 January-2014 January in the department of orthopedics in our hospital,were retrospective analyzed. After the operation,the excellent and good rate of the knee joint recovery function and operation time,bleeding quantity under operation,fracture healing time,hospital stay,complications were observed. Results 40 patients with complex tibial plateau fractures were successfully completed the operation,the excellent and good rate of the knee joint recovery function after operation was 92.5%,in which 20 cases of them recovered excellently,17 recovered nicely,2 normally,1 poorly. The operation time was（113.1±23.8） min,the amount of intraoperative bleeding was（112.85±21.90） m L,postoperative hospitalization time was（23.43±3.28） d,fracture healing time was（13.52±2.14） weeks. No case of skin necrosis,screw loosening after surgery,the incidence of complications totally was 10%. Conclusion Using locking plate internal fixation to treat patients with complex tibial plateau fracture can significantly improve the knee joint function,the fracture heal fast and less complications after operation,worthy of clinical popularization and application.
