摘要
目的通过研究阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患儿血氧指标与呼吸紊乱指标之间的相关性,为临床诊断提供参考。方法 128例经整夜多导睡眠监测(PSG)确诊为重度OSAHS的患儿,着重统计分析最低血氧饱和度(LSp O2)、血氧饱和度低于0.90占监测总时间的百分比(TS90%)与睡眠呼吸暂停低通气指数(AHI)、阻塞性睡眠呼吸暂停指数(OAI)之间的相关性。结果当AHI>20次/h时:1 LSp O2与AHI、OAI呈负相关,Pearson相关系数分别为-0.545、-0.380;2TS90%与AHI、OAI呈正相关,Pearson相关系数分别为0.779、0.645。当OAI>10次/h时:1LSp O2与AHI、OAI呈负相关,Pearson相关系数分别为-0.583、-0.418;2TS90%与AHI、OAI呈正相关,Pearson相关系数分别为0.785、0.672。以上均有统计学意义(P<0.01)。结论重度OSAHS患儿中,TS90%与AHI、OAI的相关性较LSp O2与之更强,要提高对TS90%在评估儿童缺氧程度中的关注。使用OAI指标判断疾病程度时,血氧指标与AHI及OAI相关性更强,提示儿童睡眠疾病严重程度判断中不能忽视OAI指标。
Objective To observe the clinical value of oxygen index and respiratory disorder index in pediatric obstructive sleep apnea hypopnea syndrome (OSAHS).Methods Polysomnography (PSG) was performed in 128 pediatric patients with severe OSAHS.The lowest oxygen saturation level (LSpO2),the percent of the total recorded time spent below 0.90 oxygen saturation level (TS90%),apnea hypopnea index (AHI) and obstructive apnea index (OAI) was detected and the results were analyzed statistically.Results When AHI > 20/h,①There was a negative relationship between LSpO2 and AHI,OAI.The Pearson correlation was-0.545 and-0.380 respectively; ②There was a positive relationship between TS90% and AHI,OAI.The Pearson correlation was 0.779 and 0.645 respectively.When OAI > 10/h,①There was a negative relationship between LSpO2 and AHI,OAI.The Pearson correlation was-0.583 and -0.418 respectively; ②There was a positive relationship between TS90% and AHI,OAI.The Pearson correlation was 0.785 and 0.672 respectively.There were obvious significances (P < 0.01).Conclusions The correlation between TS90% and AHI,OAI was more than that between LSpO2 and AHI,OAI in pediatric severe OSAHS.The concern of TS90% index in evaluating the degree of desaturation of children should be improved.Using OAI indicator to gauge degree of disease,the oxygen index correlation with AHI and OAI was stronger,suggesting that OAI index should not be ignored in judging the severity of pediatric sleep disease.
