健康管理家园对300名社区高血压患者血压控制情况观察

Effect of Community Health Management on Blood Pressure Control for 300 Hypertensive Patients

目的通过规范化运作健康管理家园-社区慢性病一体化管理平台,探讨其对高血压患者血压控制的的影响。方法签约全科医生的300名高血压患者进入健康管理家园-社区慢性病一体化管理平台,由全科医生进行全方位健康管理,半年后对比血压变化及高血压知晓率、治疗率和控制率情况。结果进入一体化管理平台后,高血压患者血压较前明显降低(P<0.01),高血压知晓率、治疗率和控制率均较前明显提高(P<0.01)。结论健康管理家园-社区慢病一体化管理平台可为社区高血压患者血压管理发挥重要作用。

OBJECTIVE It explored the effect of community health management operation on blood pressure control of hypertensive patients. METHODS 300 patients who signed with general practitioner were enrolled. Before and after standardized operation, the blood pressure, the rate of awareness, treatment and control were compared. RESULTS After operation of health management, the blood pressure of hypertensive patients descended obviously, the rate of awareness, treatment and control ascended obviously. CONCLUSIONS Community health management played important role in blood pressure control for community hypertensive patients.
