滚轮微针处理对复方利多卡因乳膏表面麻醉桡动脉穿刺疼痛的影响 被引量:5

Study of Microneedle Roller Treatment Influence on the Pain in Radial Artery Puncture with Compound Lidocaine Cream Topical Anesthesia

摘要 目的观察滚轮微针处理对复方利多卡因乳膏表面麻醉后行桡动脉穿刺疼痛的影响。方法选取2012年1月至2012年12月上海市第七人民医院择期手术的老年患者60例,美国麻醉师协会分级Ⅰ或Ⅱ级。按照实施方法不同分为常规组(复方利多卡因乳膏表面麻醉1 h后桡动脉穿刺)和滚轮微针组(复方利多卡因乳膏涂抹再行滚轮微针处理后15 min桡动脉穿刺)各30例。采用疼痛视觉模拟(VAS)评分评估穿刺疼痛情况,统计穿刺总成功率和穿刺满意率,穿刺后24 h皮肤不良反应发生情况。结果滚轮微针组VAS评分低于常规组[(1.1±0.4)分vs(1.9±0.5)分,P<0.05],穿刺满意率显著高于常规组(93.3%vs 70%,P<0.05);两组穿刺总成功率和皮肤不良反应(发红,淤青)比较,差异无统计学意义(P>0.05)。结论复方利多卡因乳膏涂抹后行滚轮微针处理安全性好,可减轻桡动脉穿刺疼痛,提高患者的满意度。 Objective To observe the effects of microneedle roller for pain in radial artery puncture with compound lidocaine cream topical anesthesiaof. Methods Sixty elderly patients from January to December of2012 in Shanghai Number Seven People' s Hospital with ASA Ⅰ or Ⅱ undergoing selective operation were equally randomized into two groups by random-number table method. Control group( C group = 30) received topical anesthesia of compound lidocaine cream then radial artery puncture 1h later. Microneedle group( M group = 30) received pretreatment of microneedle roller after topical anesthesia then puncture 15 min later.VAS,puncture successful rate,patient satisfaction rate,vital signs and side effects during 24 hours after operation were recorded and compared. Results VAS in the microneedle group was lower than the control group[( 1. 1 ± 0. 4) vs( 1. 9 ± 0. 5),P〈0. 05]; patients satisfaction rate was higher than the control group( 93. 3% vs 70%,P〈0. 05). There were no significant difference in puncture rate and adverse reactions( rubefaction,bruise) between the two groups( P〉0. 05). Conclusion Microneedle roller treatment is safe,which enhance the analgesic effect,reduce the puncture pain and improve patient satisfaction of compound lidocaine cream for radial artery puncture.
