照顾者反应对脑外伤患者残障接受度的影响

Effect of Caregiver Reaction to the Disability Acceptance of Traumatic Brain Injury Patients

目的了解脑外伤患者残障接受度现状及其照顾者的反应,并探讨照顾者反应对患者残障接受度的影响。方法便利选取2012年1月至2014年1月在温岭市第一人民医院脑外科就诊的221例脑外伤患者及其主要照顾者,调查患者的残障接受度及照顾者反应,采用分层回归分析法探讨照顾者反应对脑外伤患者残障接受度的影响。结果 (1)脑外伤患者残障接受度总分为(159.48±34.71)分,其中低接受度占28.51%、中接受度占54.75%、高接受度占16.74%。(2)不同残障接受度患者的健康问题、时间安排受打扰、经济问题、家庭支持缺乏和自尊评分比较,差异均有统计学意义(均P<0.05)。(3)残障接受度总分及各维度得分与照顾者反应中的健康问题、时间安排受打扰、经济问题、家庭支持缺乏均呈显著负相关(均P<0.05);残障接受度总分及各维度得分与照顾者反应中的自尊呈显著正相关(均P<0.05)。分层回归分析显示,照顾者反应中的家庭支持缺乏、经济问题、自尊和时间安排受打扰是脑外伤患者残障接受度的影响因素。结论所调查的脑外伤患者残障接受度处于中等偏下水平,照顾者反应是影响患者残障接受度的重要因素。

Objective To understand the disability acceptance and reaction of caregiver of brain injury patients,and explore the effect of caregiver reaction on the disability acceptance in patients with traumatic brain injury.Methods 221 patients with traumatic brain injury and its main caregivers were selected to explore patient＇s disability acceptance and reaction of caregivers by using hierarchical regression analysis to discuss the effect of caregiver reaction to disability acceptance in patients with traumatic brain injury.Results（1）The total score of disability acceptance was（159.48±34.71）in patients with traumatic brain injury,and 28.51% of them in the low level,54.75% of them in the medium level,16.74% of them in the high level.（2）There＇s significant difference of the score health problems,time arrangements disturbed,economic problems and lack of family support in different disability acceptance group（all P〈0.05）.（3）Disability acceptance score and score in each dimension had negative correlation with health problems,time arrangements disturbed,economic problems and lack of family support of caregivers（all P 0.05）.Disability acceptance score and score in each dimension had positive relationship with the self-esteem of reaction of caregivers（all P〈0.05）.Hierarchical regression analysis showed that lack of family support,economic problems,self-esteem and time arrangements disturbed in the caregiver reaction were the influencing factors of disability acceptance in patients with traumatic brain injury.Conclusion The level of disability acceptance was in the medium level in patients with traumatic brain injury,and caregiver reaction was the main influencing factors of disability acceptance in patients with traumatic brain injury.
