脑梗死偏瘫患者早期康复护理干预效果评价

Intervention Effect of Early Rehabilitation Nursing on Cerebral Infarction Patients with Hemiplegia

目的探讨早期康复护理干预对脑梗死偏瘫患者肢体功能恢复的效果。方法选择第三军医大学大坪医院野战外科研究所神经内科2012年1月—2013年12月收治的脑梗死偏瘫患者86例,按入院先后顺序分为常规组41例、康复组45例。常规组予脑血管病常规治疗,康复组在常规治疗基础上进行早期系统化康复护理干预。治疗前及治疗后6周采用修订的Barthel指数法进行日常生活活动能力评定,采用神经功能缺损程度评分(NDS)评定神经功能。结果两组Barthel指数、NDS治疗前差异无统计学意义(P>0.05),治疗后6周康复组Barthel指数明显高于常规组,NDS明显低于常规组,差异均有统计学意义(P<0.01,P<0.05)。结论对脑梗死偏瘫患者治疗过程中要把握康复训练时机,加强心理干预,采取正确的训练方法,以促进患者的康复,降低病后致残率,提高患者的生活质量。

Objective To investigate the intervention effect of early rehabilitation nursing on limbs function recovery of cerebral infarction patients with hemiplegia. Methods A total of 86 cerebral infarction patients with hemiplegia during January 2012 and December 2013 were divided into conventional treatment group（ n = 41） and rehabilitation group（ n = 45） according to admission time. Conventional treatment group was given conventional treatment for cerebrovascular disease,while rehabilitation group received early systemic intervention of rehabilitation nursing based on conventional treatment. The activities of daily life were evaluated before and six weeks after treatment by the revision of Barthel index method,and the nerve function was evaluated by nervous functional deficiency（ NDS） score. Results The differences in Barthel index and NDS score before treatment in the two groups were not statistically significant（ P〉 0. 05）; in rehabilitation group the Barthel index was significantly higher,while NDS score was significantly lower six weeks after treatment compared with those in conventional treatment group,and the differences were statistically significant（ P〈0. 01,P〈 0. 05）. Conclusion In the process of treatment of cerebral infarction patients with hemiplegia,physicians should take opportunities of rehabilitation training,strengthen the psychological intervention,take the right training method in order to promote the recovery of patients,reduce the mutilation rate after illness and improve the patient＇s quality of life.
