MSCT扫描在先天性心外大血管畸形中的应用价值 被引量:1

摘要 目的探讨多层螺旋CT(MSCT)在先天性心外大血管畸形中的诊断价值。方法分析具有典型影像学表现或经手术证实的35例先天性心外大血管畸形患者胸部MSCT增强扫描及经胸超声心动图(UCG)检查资料,其中27例同时进行CTA扫描。比较MSCT结果与UCG结果,MSCT增强结果与CTA结果。结果先天性心外大血管畸形共86处。MSCT、UCG符合率分别为95.3%和79.1%,两者比较差异有统计学意义(2=8.45,P<0.01);MSCT增强结果与CTA结果符合率分别为80.2%和95.7%,两者比较差异有统计学意义(2=7.69,P<0.05)。结论 MSCT尤其是MSCTA,在先天性心外大血管畸形的诊断中具有明显优势,可以全面清晰地显示先天性心外大血管畸形的病理解剖结构,为诊断、治疗及预后提供了可靠的依据。 Objective It is to investigate the value of multi-slice spiral CT ( MSCT) in the diagnosis of congenital extracardiac vascular malformation.Methods Congenital extracardiac vascular malformation was analyzed in 35 patients which had typical iconographical performances or been confirmed by surgery.All the patients were examined by contrast-enhanced MSCT and by ultrasound cardiograms ( UCG) , 27 of them underwent computed tomography angiography( CTA) .Then according to the results of clinical conclusions,all comprehensive informations of MSCT were compared with the findings of UCG,the results of contrast-enhanced MSCT were compared with the results of CTA.Rseults A total of eighty-six extracardial vascular mal-formations were confirmed.The definite rate of diagnosis of MSCT and UCG were 95.3%,79.1%respectively,and the difference was statistically significant(χ^2=8.45,P〈0.01 ) .The definite rate of diagnosis of contrast-enhanced MSCT and CTA were 80.2%,95.7%respectively,and the difference was statistically significant(χ^2=7.69,P〈0.05).Conclusions MSCT has obvious advantages in diagnosis of patients with congenital extracardiac vascular malformation,especially the multi-slice spiral CTA which systematically and clearly demonstrates the pathological morphology of them, and provides reliable supports for the diagnosis,the treatment and the prognosis.
