自制表带式止血带在腹腔镜子宫肌瘤剔除术中的价值研究

The value of the homemade watchband type tourniquet in laparoscope hysteromyomectomy

目的探讨自制表带式止血带在腹腔镜子宫肌瘤剔除术中的实用性、便捷性及安全性。方法选择2010年9月-2013年6月在漯河医学高等专科学校第一附属医院行腹腔镜子宫肌瘤剔除术的子宫肌瘤患者100例,完全随机分为A组(实验组)和B组(对照组),A组52例,术中应用自制表带式止血带,B组48例,常规手术,比较两组术中出血量、手术时间和术后病率。结果实验组1例因盆腔致密粘连转为开腹手术,其余99例均在腹腔镜下完成,其中B组输血2例。A组平均出血量、术后病率均低于B组,差异有显著性(P<0.05),两组平均手术时间差异无显著性。结论自制表带式止血带阻断子宫供应血管在腹腔镜子宫肌瘤剔除术中能减少术中出血量、术后病率,利于术后恢复,是腹腔镜子宫肌瘤剔除术中方便、有效而且安全的止血方法。

[ Objective ] To explore the practicality, convenience and safety of homemade watchband type tourniquet in laparoscopic hystero-myomeetomy. [Methods]100 patients with laparoseope hystero-myomectomy between September 2010 and June 2013 in The First Affiliated Hospital of Luohe Medical College were recruited to this study. They were completely randomly divided into group A （experimental group） and group B （control group）, A group of 52 cases, intraoperative application of homemade watchband type homemade tourniquet, B group of 48 cas- es, conventional surgery, intraoperative blood loss, operative time and postoperative disease rate were compared between the two groups. [ Results ] One case was converted to laparotomy because of the dense pelvic adhesions in experimental group, the remaining 99 eases were performed laparoseopie. Two cases were performed blood transfusion in control group. Average blood loss, postoperative disease rate in group A was different from group B （P〈0.05）, the average operation time similar between the two groups had no statistical significance. [ Conclusion ] The homemade watchband type tourniquet in laparoseope hystero-myomectorny can reduce intraoperative blood loss, postoperative disease rate, promote postoperative recovery, it is a kind of convenient, safe and effective hemostatic method.
