双侧脂肪筋膜瓣治疗坐骨结节Ⅳ度压疮

Double adipofascial turnover flaps in repairing stage Ⅳ ischial pressure scores

目的 探讨利用双侧脂肪筋膜瓣结合负压引流,治疗坐骨结节Ⅳ度压疮的临床疗效.方法 根据压疮创面情况和脂肪筋膜瓣血供丰富特点,设计创面两侧脂肪筋膜瓣,翻转重叠覆盖创面,并结合负压引流,治疗11例坐骨结节Ⅳ度压疮.结果 11例创面均一期完全愈合,经术后11～36个月随访,原手术部位无压疮复发,外观良好.结论 双侧脂肪筋膜瓣结合负压引流治疗坐骨结节Ⅳ度压疮,方法简单安全,疗效满意,值得推广。

Objective To report our therapeutic effect of double adipofascial turnover flaps in repairing stage Ⅳ ischial pressure scores.Methods During the period of May 2009 to February 2013,we treated 11 cases of stage Ⅳ ischial pressure scores with double adipofascial turnover flaps and proper drainage.Based on the conditions of pressure scores and abundant blood supply of adipofascial flaps,we designed two adipofascial flaps on each side of lesion.The two flaps were overlaped to cover the pressure score,following with negative pressure drainage.Results All pressure scores healed primarily.The follow-up period ranged from 11 to 36 months with satisfactory results and no recurrence.Conclusions Double adipofascial turnover flaps and proper drainage is a safe and simple method for the treatment of stage Ⅳ ischial pressure scores with satisfactory results.
