右美托咪定在AECOPD合并Ⅱ型呼吸衰竭机械通气中的应用被引量：3

摘要目的评估右美托咪定在慢性阻塞性肺疾病急性加重(AECOPD)合并Ⅱ型呼吸衰竭机械通气中的镇静效果。方法选择AECOPD合并Ⅱ型呼吸衰竭行机械通气的患者52例,随机均分为右美托咪定治疗组和咪达唑仑对照组。观察两组患者用药前后呼吸相关生理指标及临床指标的变化。结果两组镇静效果相似(P>0.05)。治疗组拔管时患者无明显不适,且生命体征较平稳;对照组拔管时患者出现烦躁不安,且HR、MAP、RR均较拔管前有明显变化(P<0.05)。治疗组谵妄发生率、48h内再插管率低于对照组,且平均插管时间、ICU留住时间均较对照组短(P<0.05)。结论右美托咪定用于AECOPD合并Ⅱ型呼吸衰竭机械通气镇静效果确切,不良反应低,ICU留住时间短。
