血液透析联合血液灌流对维持性血液透析患者微炎性因子及钙磷代谢的影响 被引量:41

摘要 目的:探讨血液透析联合血液灌流对维持性血液透析患者微炎性因子及钙磷代谢的影响。方法:选取在本院接受维持性血液透析治疗的患者共116例,将其随机分为观察组和对照组,各58例。其中对照组采取血液透析治疗,观察组采取血液透析与血液灌流联合治疗。观察并比较两组治疗前后炎性因子水平变化及钙磷代谢改善情况。结果:观察组治疗后白介素-6(IL-6)、超敏C-反应蛋白(hsCRP)、肿瘤坏死因子-α(TNF-α)及血磷、成纤维细胞生长因子-23(FGF-23)、全段甲状旁腺素(iPTH)水平与对照组比较明显降低,差异均具有统计学意义(P<0.05);而血红蛋白(HB)、血清白蛋白(Alb)及血钙、血清1,25-(OH)2D3水平与对照组比较明显升高,差异均具有统计学意义(P<0.05)。结论:血液透析联合血液灌流对维持性血液透析患者具有更好的临床疗效,可以有效地改善患者的微炎症状态,维持其钙磷代谢平衡,进而提高患者的生活质量和生存率,值得大力推广和应用。 Objective:To investigate the effects of hemodialysis combined with hemoperfusion on micro inflammatory factor and calcium phosphorus metabolism of patients with maintenance hemodialysis.Methods:A total of 116 cases with maintenance hemodialysis were collected and randomly divided into the observation group and control group according to the order of treatment,each group consisted of 58 cases.Patients in the control group were given blooddialysis treatment,the observation group patients were given hemodialysis combined with hemoperfusion treatment.The changes of calcium and phosphorus metabolism of inflammatory factor level in two groups were observed and compared before and after treatment.Results:IL-6,hsCRP,TNF-alpha and serum phosphorus,FGF-23,iPTH levels of patients in the observation group were decreased significantly comparing with the control group(P〈0.05);while the HB,Alb and serum calcium,serum 1,25-(OH)2D3levels increased significantly compared with the control group(P〈0.05).Conclusion:Hemodialysis combined with hemoperfusion shows better clinical therapeutic effect on patients with maintenance hemodialysis,it can effectively improve the micro inflammatory state,maintain the metabolism of calcium and phosphorus balance,and improve the patient’s quality of life and survival rate,thus it is worthy of popularizing and application.
