经后路改良楔形截骨治疗创伤后胸腰椎骨折后凸畸形 被引量:6

Correction of posttraumatic kyphosis of the thoracolumbar spine with modified pedicle subtraction osteotomy

摘要 目的观察后路经椎弓根椎体椎间隙楔形截骨治疗创伤后胸腰椎骨折后凸畸形的临床疗效。方法自2011-01-2014-01共22例创伤后胸腰椎骨折脊柱后凸畸形患者采用改良PSO截骨矫形技术治疗,所有患者均有严重的胸腰区慢性疼痛,VAS评分≥7分,无神经功能障碍。结果 1例术中发生硬膜撕裂,无神经血管损伤。在Cobb角,VAS评分和ODI评分方面,术后两次测量结果较之术前,差异均有统计学差异(P<0.05),所有病例截骨面均骨性愈合。结论经椎弓根椎体椎间隙楔形截骨矫正创伤后胸腰椎骨折后凸畸形是一种有效的手术方法,截骨面融合率高,并发症发生率低。 Objective To observe the clinical curative effect of pedicle vertebral intervertebral disc wedge bone cutting in the treatment of post-traumatic thoracolumbar fractures protrusion deformity. Methods From January 2011 to January 2014,22 cases of post-traumatic thoracolumbar spine fracture after protrusion deformity were treated by modified PSO bone cutting orthopaedic treatment technology. All patients had severe chest waist area of chronic pain,VAS score more than7 points,and without nerve dysfunction. Results 1 case was found hard film tear,no nerve vascular injury. The results of two measurements in Cobb angle,VAS score and ODI scores after operation had statistically significant differences compared with preoperation(P〈0.05),all cases bone cutting surface osseous healed. Conclusion The pedicle vertebral intervertebral disc wedge bone cutting convex deformity after correction post-traumatic thoracolumbar fractures is an effective surgical method,it has high fusion rate of bone cutting face and low incidence rate of complications.
