p16基因甲基化与肝细胞癌易感性的荟萃分析

摘要 目的通过荟萃(Meta)分析系统评价抑癌基因p16启动子区异常甲基化与肝细胞癌易感性的关系。方法利用Pubmed、Embase、Ovid、Web of science、Cochrane Library等数据库,中国生物医学文献数据库(CBM)、中文科技期刊全文数据库(VIP)、中国期刊全文数据库(CNKI)、万方数据库检索2014年6月以前的相关文献,按纳入排除标准筛选文献并提取数据,用Rev Man5.2软件统计分析。结果共纳入研究23项累计病例2 245例,其中肝细胞癌组1 106例,对照组1 139例(包括癌旁组织402例、肝硬化239例、肝炎39例、正常组织141例,肝结节22例以及非癌组织296例)。p16基因甲基化阳性率在肝细胞癌组织中均显著高于正常肝组织[OR=21.18,95%CI(9.75-46.00),P〈0.05]、非癌组织[OR=7.97,95%CI(4.64-13.68),P〈0.05]及肝硬化组织[OR=6.18,95%CI(4.16-9.18),P〈0.05]。结论 p16基因高甲基化与肝细胞癌易感性密切相关。 Objective To evaluate the association of p16 promoter methylation with the risk of hepatocellular carcinoma(HCC) by sys- tematic meta-analysis. Methods The literatures related to the p16 methylation in HCC were retrieved from Pubmed, Embase, Ovid, Web of science, Cochrane Library, CBM, VIP, CNKI and Wanfang data base before June 2014. Data were screened according to the inclusion and exclusion criteria and analyzed by RevManS. 2 software. Results A total of 2 245 cases from 23 references were in- volved in the meta-analysis which composed of 1 106 HCC cases and 1 139 controls, including 402 cases of pericarcinous tissue, 239 cases of cirrhosis, 39 cases of hepatitis, 141 cases of normal tissue, 22 cases of nodule tissue and 296 cases of non-carcinoma tissue. The methylation rate ofpl6 gene in HCC tissues was significantly higher than those of normal liver tissues[ OR = 21.18,95% CI(9.75- 46.00 ), P 〈 0.05 ], non-carcinoma tissue [ OR = 7.97,95 % CI ( 4.64-13.68 ), P 〈 0.05 ] and cirrhosis tissues [ OR = 6.18,95 % CI (4.16-9.18) ,P 〈0.051. Conclusion The present meta-analysis indicates that the methylation ofpl6 promoter should be closely associated with the susceptibility of HCC.
