摘要
目的研究引起血流感染大肠埃希菌的耐药谱及系统发生分型,为临床治疗提供指导依据。方法收集2007-2010年医院血流感染大肠埃希菌的非重复菌株,进行临床分布统计、耐药谱测定,并用多重PCR方法对菌株进行系统发生分型检测。结果 271株大肠埃希菌对头孢他啶、头孢噻肟、头孢西丁的敏感率分别为64.9%、36.2%、68.3%;检出产ESBLs菌164株占60.5%,非产ESBLs菌107株占39.5%;系统发生分型以D群和B2群为主,分别占41.0%、32.5%,A群和B1群比例较少,分别占17.0%、9.5%。结论血流感染大肠埃希菌4年的耐药谱未发生明显变化,对碳青霉烯类抗菌药物,阿米卡星敏感率较高,系统发生分型以B2和D群为主。
OBJECTIVE To study the phylogenetic group and drug resistance of Escherichia coli causing blood‐stream infection so as to guide clinical treatment .METHODS A total of 271 non‐repeated strains of E .coli were collected from our hospital from 2007 to 2010 .The drug resistance and clinical distribution of the strains were ana‐lyzed ,and the phylogenetic type was detected by multiple PCR .RESULTS The susceptibility rate to ceftazidine , cefotaxime and cefoxitin were 64 .9% ,36 .2% and 68 .3% ,respectively .Among 271 strains ,60 .5% of isolates produced ESBLs ,41 .0% and 32 .5% of isolates were identified belonging to phylogenetic groups D and B2 ,while only 17 .0% and 9 .5% belonged to groups A and B1 .CONCLUSION During the four years ,the resistance profile of E .coli did not change significantly .The susceptibility rates to Carbapenems and Amikacin were very high .The phylogenetic groups of the stains were mainly groups B2 and D .
