血栓抽吸在低血栓负荷急性ST段抬高心肌梗死介入治疗中的应用 被引量:5

摘要 目的:比较血栓抽吸联合直接经皮冠状动脉介入治疗(PCI)与单纯PCI在低血栓负荷的急性ST段抬高心肌梗死(STEMI)患者中的应用,探讨直接PCI的最佳策略。方法:回顾分析79例因STEMI行直接PCI患者的临床资料。根据术中对血栓处理的不同分为单纯PCI组(n=38)和联合抽栓组(n=41)。比较两组术后慢血流或无复流发生率、手术时间、直接支架植入术例数、术后1周和3个月射血分数及左室舒张末内径、住院期间心血管死亡率。随访3个月再发心肌梗死、血运重建、心力衰竭及心血管死亡率。结果:联合抽栓组慢血流或无复流的发生率有降低趋势(2.4%vs.7.9%,P>0.05),但手术时间明显延长[(73.10±18.15)min vs.(63.34±12.58)min,P<0.05];其余指标比较差异无统计学意义。结论:对低血栓负荷急性STEMI患者,直接PCI术中常规抽栓,虽然慢血流或无复流发生率有降低趋势,但住院期间及3个月随访无明确临床获益。
