PCOS患者早卵泡期子宫内膜血流动力学参数与内分泌参数的相关性分析 被引量:9

Analysis on the correlation between hemodynamic parameters of endometrium in PCOS patients during early follicular phase and endocrine parameters

摘要 目的:通过检测PCOS患者早卵泡期子宫内膜周围螺旋动脉的血流动力学参数分析其与临床代谢、基础内分泌参数之间的相关性。方法:取44例闭经期PCOS患者于早卵泡期行TV-CDU检查,检测内膜周边螺旋动脉彩色血流特征及血流参数-搏动指数(PI)及阻力指数(RI);同期测定各研究对象身高、体重以及性激素6项,各参数之间采用Pearson相关系数分析。结果:血清LH与BMI、WHR均显著负相关(P<0.01),LH/FSH比值与BMI、WHR也均显著负相关(P<0.01);PCOS患者早卵泡期子宫内膜周围螺旋动脉PI与血清T水平显著正相关(P<0.01),RI与LH/FSH正相关(P<0.05)。结论:TV-CDU检测内膜周围螺旋动脉血流动力学为评定PCOS的子宫内膜情况提供了重要的临床应用价值,且子宫内膜血流情况受PCOS患者内分泌状况的影响。 Objective: To analyze the correlation between hemodynamic parameters and clinical metabolism,basic endocrine parameters by detecting hemodynamic parameters of spiral arteries around endometrium in polycystic ovary syndrome( PCOS) patients during early follicular phase. Methods: TV-CDU examination was performed among 44 PCOS patients with menopause during early follicular phase,blood flow characteristics and blood flow parameters around endometrium─pulsatility index( PI) and resistance index( RI) were examined; body height,body weight and six items of sex hormones were measured during the same period; Pearson correlation coefficient analysis was used to analyze the correlation among the parameters. Results: Serum luteinizing hormone( LH) level was negatively correlated with body mass index( BMI) and waist hip rate( WHR)( P 0. 01),the ratio of LH / follicle-stimulating hormone( FSH) was negatively correlated with BMI and WHR( P 0. 01); PI was positively correlated with serum testosterone level( P 0. 01),RI was positively correlated with the ratio of LH / FSH( P 0. 05). Conclusion: Detecting hemodynamic parameters of spiral arteries around endometrium by TV-CDU has important clinical application value for assessment of endometrial situation in PCOS patients,and endometrial blood flow is affected by endocrine situation of PCOS patients.
