角膜后弹力层脱离的原因及处理

The Cause and Treatment of Corneal Posterior Membrane Detachment

目的探讨引起角膜后弹力层脱离的原因及处理方法。方法回顾性分析25例28眼各种原因造成的角膜后弹力层脱离患者的临床资料。结果白内障囊外摘除(ECCE)及青光眼白内障联合手术是引起角膜后弹力层脱离的常见原因,前房注气后25例患者的28眼角膜后弹力层脱离均复位,随访3个月后28眼角膜均透明。结论超声乳化白内障摘除(PHACO)/ECCE+人工晶状体植入(IOL)术术中角膜后弹力层脱离病例注入过滤空气气泡进行复位,青光眼白内障联合手术术中角膜后弹力层脱离病例注入黏弹剂于小梁切除部位并结合前房注入气泡进行复位,二者复位效果确切,值得临床推广。

Objective To investigate the causes and treatment of corneal posterior membrane detachment.Method The clinical data of 25 cases of 28 eyes were retrospectively analyzed the various causes of corneal posterior membrane detachment.Results The common causes of corneal posterior membrane detachment were extra-capsular cataract extraction（ECCE）and glaucoma-cataract surgery.28 cases of corneal posterior membrane in 25 patients were reset after anterior chamber air injection and 28 corneas were all transparent after three-month follow-up.Conclusion Filtered air bubble injection in the operation of Phacoemulsification（PHACO）/ECCE and intraocularlens（IOL）implantation helps corneal posterior membrane detachment to reset.The injection of viscoelastics in trabeculectomy site and the anterior chamber air bubble injection in the glaucoma-cataract surgery can make corneal posterior membrane detachment reset.Both have positive effects,worthy of clinical promotion.
