术前肠内营养对食管癌患者医院感染的临床研究

Cinical research of influence of preoperative enteral nutrition to esophageal cancer patients with hospital infections

目的观察分析术前肠内营养对低肺功能食管癌患者医院感染影响,以探讨降低医院感染发生率的预防措施。方法选取医院从2010年3月-2012年3月收治的食管癌患者共120例,随机将患者分为观察组和对照组,每组各60例;观察组患者采用术前肠内营养支持,对照组患者则采用常规静脉营养支持,比较两组患者治疗过程中医院感染发生情况、血气指标、总蛋白、清蛋白、血红蛋白等指标的变化,采用SPSS17.0进行统计分析。结果观察组患者治疗过程中共有2例发生医院感染,发生率为3.33%;对照组有6例发生医院感染,发生率为10.0%;观察组患者在接受治疗后血总蛋白、清蛋白、血红蛋白均较治疗前有明显上升,但观察组患者上升幅度明显高于对照组,组间上升幅度比较,差异均有统计学意义(P<0.05)。结论术前肠内营养支持治疗能够有效改善患者的营养状况,提高血气指标,提高抵抗力,降低患者医院感染发生率,为手术治疗提供坚实保障。

OBJECTIVE To observe the influence of preoperative enteral nutrition to esophageal cancer patients with hospital infections ,so as to reduce the incidence of hospital infection .METHODS A total of 120 patients with esophageal cancer were selected from Mar .2010 to Mar .2012 in the hospital .Patients were randomly divided into observation group and control group ,with 60 cases in each group .The observation group were treated with preop‐erative enteral nutrition ,while the control group received the conventional intravenous nutritional support ,the two groups were compared with hospital infection occurrence ,qi and blood index ,total protein ,albumin ,hemoglobin , etc index changes .SPSS 17 .0 software was adopted for a statistic analysis .RESULTS Totally 2 cases in the obser‐vation group had hospital infections ,with the incidence rate of 3 .33% ;totally 6 cases in the control group had occurred hospital infections ,with the incidence rate of 10 .0% ;after the treatment ,the blood total protein ,the albumin ,and the hemoglobin of the two groups were improved significantly ,but the improving degree in the observation group was significantly higher than that of the control group （P＆lt;0 .05） .CONCLUSION Preoperative enteral nutrition support treatment can effectively improve the nutritional status of patients ,by improving the blood indicators ,and resistance ,reducing the incidence of hospital infection and providing a solid guarantee for the surgical treatments .
