腰-硬膜外联合麻醉下剖宫产产妇吸入性肺炎的发生情况 被引量：5

Incidence of pulmonary aspiration in parturients after cesarean section with combined spinal-epidural anesthesia

摘要 产妇吸入性肺炎作为一个产科麻醉中的并发症首先在1946年由门德尔松描述[1],亦称为门德尔松综合征,其定义为吸入胃内容物导致的急性化学性肺损伤.吸入的原因为除全麻后气道防御反射下降外,还存在妊娠及生产过程中的腹内压增加加全麻肌松药物使用导致的胃肠道蠕动减少致使胃内液体返流吸入.随着医学的进展,临床上目前认为吸入性肺炎还包含了吸入有致病菌繁殖的口咽分泌物引起的肺部炎症,常为隐性吸入,其病原菌常见为厌氧菌及革兰阴性杆菌[2-3].吸入性肺炎常伴随明显的临床症状与体征,严重者可进展为急性呼吸窘迫综合征（ARDS）[4-6].
