商宪敏教授中药辨证治疗腰椎间盘突出症的临床经验

Professor SHANG Xian-min’s experience on lumbar disc herniation by using herbs based on syndrome differentiation

商宪敏教授认为腰椎间盘突出症多以正气亏虚,尤其肾虚骨弱为其内因,外感风、寒,湿邪气,跌扑外伤及劳损为其诱因,气血运行不畅、经络闭阻则是该病的主要病机。分为寒湿痹阻型、湿热痹阻型、血瘀脉阻型、肝肾亏虚型4型进行中药辨证治疗。总结出以补肾壮骨、散寒祛风、除湿通络、活血化瘀为主的通用基础方,并根据兼夹症的不同在临床中进行药物的加减化裁,临床取得了较好的疗效。

Professor SHANG Xian-min points out that the basic internal etiology of lumbar disc herniation （LDH） is vital energy deficiency, especially the kidney deficiency and bone frailty, with the precipitating factors like wind, cold and dampness pathogenic qi, traumatic injuries and strain. The major pathogenesis of LDH is qi and blood could not be circulated smoothly, then, meridians and collaterals are blocked. Four main syndrome types of LDH are put forward by professor SHANG Xian-min, including cold-dampness stasis syndrome, dampness-heat stagnant syndrome, blood stasis syndrome and syndrome of yin deficiency of liver ＇and kidney. She sums up a general prescription with the function of tonifying kidney and strengthing bones, expelling coldness and wind, clearing dampness for dredging collaterals, and promoting blood circulation for removing blood stasis. Furthermore, the prescription could be added or subtracted in clinic according to different concurrent syndromes.
