目的 :开发肿瘤患者出院延续性护理信息平台并应用。方法 :通过联结医院信息系统(HIS)、门诊诊疗系统等多个模块的专用软件,摘取或移植患者基本信息及有关肿瘤学诊治的相关信息,建立以医院为基础的肿瘤患者出院延续性护理数据库。结果 :该信息平台易于操作、实用性强,提供更高质量的肿瘤病例信息资料,确保信息的完整性、正确性。结论 :信息技术的开发运用,为肿瘤患者出院延续性护理的开展提供了保障,同时为进行相关研究提供了数据。

Objective： To establish and application post-discharged continuity of care information platform for cancer patients. Methods： By coupling the special software of multiple modules such as the hospital HIS, outpatient clinic system, and extracting the basic information and oncology-related diagnosis and treatment information to establish a hospital-based postdischarged continuity of care information platform for cancer patients. Results： The information platform with practicality was easy to operate, and provided higher quality information material of the cancer cases to ensure information integrity and correctness. Conclusion： The development and application of information technology provide a guarantee for the carrying out of post-discharged continuity of care information platform for cancer patients, as well as providing a basis for cancer research.
