血清胱抑素C在窒息新生儿肾损害中的诊断价值 被引量:6

摘要 目的探讨血清胱抑素(SCys C)对窒息新生儿肾功能损伤的诊断价值及临床意义。方法将符合窒息诊断标准的新生儿75例,按新生儿窒息Apgar评分标准分为轻度窒息组(55例)和重度窒息组(20例);同期选择40例健康足月新生儿作为对照组。所有观察对象在日龄24小时内采取外周静脉血2ml,检测SCys C、血清肌酐(SCr)、血尿素氮(BUN)水平,比较各组值的差异。结果①对照组SCys C浓度为(1.70±0.20)mg/L,SCr为(63.17±13.45)μmol/L,BUN为(3.42±1.50)mmol/L;②轻度窒息组和重度窒息组SCys C与对照组比较,差异有非常显著性(P<0.001);重度窒息组SCr较对照组显著增高(P<0.05);而在轻度窒息组与对照组差异无显著性(P>0.05);BUN三组间比较差异无显著性(P>0.05);③SCys C和SCr在轻度窒息组的异常率分别为47.27%和10.9%,差异有非常显著性(P<0.01);在重度窒息组的异常率分别为75%和40%,差异有显著性(P<0.05);SCys C和BUN在轻度窒息组和重度窒息组的异常率(14.54%、10%)差异有非常显著性(P<0.01)。结论①窒息可造成新生儿肾功能受损,且窒息程度越重,肾功能损伤的程度也越重;②SCys C可作为检测窒息新生儿肾功能损伤的早期指标,且敏感性优于SCr、BUN;③SCys C在足月新生儿的可能范围是(1.70±0.20)mg/L(测定方法:PETIA)。
