摘要
目的应用扩散张量成像(DTI)分析健康志愿者股外侧肌延迟性肌肉酸痛(DOMS)影像学改变及其与肌肉酸痛指数之间的相关性,探讨DTI在评价DOMS中的应用价值。方法 9名健康志愿者运用短距离重复跑运动模式进行训练以获得DOMS模型,在运动前、运动后的1 d、2 d、3 d、5 d对左侧股四头肌进行DTI扫描,并行肌肉酸痛指数评分。测量左侧股外侧肌感兴趣区ADC值、FA值及本征值。运用重复测量资料的方差分析对各指标不同时间上的变化的特点。用Spearman秩相关分析对DTI测量结果与肌肉酸痛指数进行相关性分析。结果 9名健康志愿者左侧股外侧肌DTI检查显示左侧股外侧肌的ADC值在运动后24 h逐渐增高,48 h达到高峰,72 h后逐渐降低,5 d时逐渐恢复正常。FA值和λ3亦在运动后24 h^48 h逐渐增高,48 h时达最大值,后逐渐降低,5 d时恢复正常,λ1,λ2亦显示出同样的变化趋势。统计学分析显示FA、λ3与肌肉酸痛指数有一定相关性(r值分别为0.577、0.214)。结论 DTI能够有效评估DOMS后骨骼肌损伤程度改变,DTI与酸痛指数能够对DOMS进行综合评价,为运动训练提供科学的理论依据和指导。
Objective To analyze the vastus lateralis changings in MR-DTI of healthy volunteers with delayed onset muscle soreness (DOMS) and evaluate the correlation between metabolic changings in DTI parameters with serological markers related to DOMS of skeletal muscle. Methods Nine healthy volunteers [ aged (24.9±0.7) years, height ( 168.7±4.8 ) era, weight (64.2±6.0) kg ] were recruited for the study, and conducted a mo- tion model to induce DOMS. DOMS and MR scan were carried out immediately before and at 1 day, 2 day, 3 day, 5 day after exercise to elucidate the extent of muscle damage. Repeated measures ANOVA (analysis of variance)was used for DTI parameters of each time, including the FA, ADC, and eigenvalues. Correlation between index of Muscle soreness and DTI parameters was assessed with Spearman rank correlation analysis. Results The FA and k3 values gradually increased after exercise, reaching their maximum in 24-48 hours after exercise in time consistency, and gradually re- turned to the pre-exereise level. After exercise, the ADC values can be reaching their maximum in 48 hours after exercise, and then gradually re- turned to the pre-exereise level. Muscle soreness were significantly elevated above baseline for 48 hours post-exercise ; the muscle soreness gradually returned to pre-exercise level five days after exercise. There are some correlations among FA, k3 and index of muscle soreness (r =0.577,0.214). Conclusion DTI could quantitatively reflect the human exercise-induced muscle damage after high intensity exercise, which corresponding to mus- cle soreness changes ; the FA and k3 values could evaluate the degree of injury, which also can be taken as indexes of exereise-induced Sore-pain.
