摘要
目的探讨特制背心配合早期活动方案预防心脏永久起搏器植入术后上肢深静脉血栓(upper extremity deep venous thrombosis,UEDVT)的效果及对电极安全的影响。方法 2011年8月至2014年3月,在上海市第七人民医院心内科行心脏永久起搏器植入手术的患者150例,按随机数字表法分为3组,实验1组、实验2组、对照组各50例。实验1组术后穿特制背心,遵循早期活动方案活动;实验2组采用起搏器植入术后护理常规,术后增加穿特制背心,72h后下床活动;对照组采用起搏器植入术后护理常规,不穿背心,72h后下床活动。评价并比较3组患者UEDVT及电极脱位发生率的差异。结果 3组患者UEDVT发生率的差异有统计学意义(P<0.05),其中实验1组的发生率最低;3组患者均未发生电极脱位。结论特制背心配合早期活动方案能有效降低UEDVT的发生率,且并不增加电极脱位发生率,值得在临床推广应用。
Objective To discuss the effect of special designed vest in corporate early stage activity in pre‐vention upper extremity deep venous thrombosis (UEDVT) after patients had cardiac pacemaker implanta‐tion and its influence of electrode security .Methods Totally 150 patients with cardiac pacemaker implanta‐tion were selected and randomly divided into experimental Group 1 ,experimental Group 2 and control group with 50 cases in each group .Patients in Group 1 weared the special designed vest after the implanta‐tion;those in Group 2 were treated with conventional nursing after the implantation ,and also weared the special designed vest ,and can ambulate after 72 hours;those in the control group were treated with conven‐tional nursing after the implantation without wearing the vest ,and can ambulate after 72 hours .Then com‐pare the difference in the incidence rate of the UEDVT and the electrode dislocation between the 3 groups . Results There was statistical significance of UEDVT incidence rate between 3 groups (all P〈0 0.5) ,and the Group 1 had the lowest incidence of UEDVT .Furthermore ,no group had electrode dislocation .Conclu‐sion T he special designed vest gives assistance in the early activity program w hich can effectively reduce the UEDVT incidence rate and ensure the safety of the electrode ,and it’s worthwhile for clinical trial .
