摘要
目的探讨Leptin和血管内皮生长因子(vascular endothelial growth factor,VEGF)与乳腺癌临床病理特征的关系,以及Leptin和VEGF对乳腺癌总体生存率的影响。方法选取包含生存资料的159点乳腺癌组织芯片,免疫组织化学方法测定组织芯片中Leptin和VEGF的表达。统计分析Leptin与VEGF在乳腺癌与癌旁组织中的表达差异,Leptin与VEGF乳腺癌组织中表达的相关性,以及它们与乳腺癌临床病理特征相关性和对乳腺癌总体生存率的影响。结果乳腺癌组织Leptin(χ2=7.564,P=0.006)与VEGF(χ2=9.685,P=0.002)表达均明显高于癌旁组织。Leptin表达与肿瘤大小、淋巴结转移和临床TNM分期呈正相关,P值均<0.05;VEGF表达与淋巴结转移和临床TNM分期呈正相关,P值均<0.05。Leptin高表达组和Leptin-VEGF同时高表达组乳腺癌病例的总体生存率分别较Leptin低表达组和Leptin-VEGF非同时高表达组明显降低。结论 Leptin表达是乳腺癌患者预后的独立因素,VEGF可能在Leptin介导的乳腺癌发生发展中起重要作用。
OBJECTIVE To find out the correlation between Leptin and clinicopathological parameters of breast cancer, vascular endothelial growth factor(VEGF) and clinicopathological parameters of breast cancer, Leptin and VEGF respectively,and identify clinical significance of Leptin and/or VEGF expression on overall survival of breast cancer; METHODS Immunohistochemistry (IHC) was performed on paraffin-embedded tissue microarray containing 159 tissue dots from breast cancer patients. The intensity and the extent of IHC were scored by pathologists blind to clinicopatholog- ical parameters of the specimens. The correlation between Leptin and VEGF, Leptin and clinicopathological parameters, VEGF and clinicopathological parameters were analyzed for statistical significance and the impact of Leptin and VEGF ex- pression on overall survival of breast cancer. RESULTS Both Leptin and VEGF expression were significantly higher in breast cancer tissues than in their corresponding paraneoplastic tissues (X^2 = 7. 564, P = 0. 006 ; X^2 = 9. 685, P= 0. 002). Expression of Leptin in breast cancer positively correlated with tumor size, lymphma node metastasis, clinical TNM clas sification (all P〈0.05). Expression of VEGF in breast cancer positively correlated with lymphma node metastasis and clinical TNM classification (all P〈0.05). High Leptin expression group and high Leptin-VEGF expression group tend to have lower overall survival than low Leptin expression group and non-high Leptin-VEGF expression group. CONCLU- SIONS High Leptin expression is more common in breast cancer tissues than in their corresponding paraneoplastic tis- sues, and is an independent prognosis indicator of breast cancer. VEGF may play a pivotal role in the development of Lep- tin-induced breast cancer.
