鼻塞持续正压通气治疗重症肺炎患儿的临床疗效及对患儿肺氧合功能的影响

The Clinical Efficacy of Continuous Positive Airway Pressure Therapy in Children with Severe Pneumonia and its Effects on Oxygenation Function

目的:评价鼻塞持续正压通气治疗重症肺炎患儿的临床疗效及对患儿肺氧合功能的影响,探讨其临床适用性。方法:选择2012年5月-2013年5月于本院就诊的98例重症肺炎患儿,简单随机分为试验组49例和对照组49例,对照组患者采用头罩吸氧治疗,试验组采用鼻塞持续正压通气治疗。观察两组患者治疗后的临床疗效和肺氧合功能变化情况,观察并记录两组患者的用氧天数、吸入氧体积分数(Fi O2)、住院天数。结果:试验组治疗后总有效率为91.8%(45/49),对照组为77.6%(38/49),两组临床治疗总有效率比较,差异有统计学意义(P<0.05),试验组治疗后Pa O2、动脉血氧分压/肺泡氧分压比值[a/Ap(O2)]高于对照组,差异均有统计学意义(P<0.05);而试验组氧合指数(Oi)低于对照组,差异均有统计学意义(P<0.05),试验组的用氧天数、吸入氧体积分数(Fi O2)、住院天数均低于对照组患者,两组比较差异均有统计学意义(P<0.05)。结论:鼻塞持续正压通气治疗重症肺炎患儿临床疗效显著,且可以显著改善患者肺氧合功能,缩短住院时间,适合临床长期推广应用。

Objective：To analyze the clinical efficacy of continuous positive airway pressure therapy in children with severe pneumonia and its effects on oxygenation function.Method：98 cases of severe pneumonia children were simply randomly divided into experimental group and control group,oxygen hood was used in the control group,while the nasal continuous positive airway was used in the experimental group pressure.The clinical efficacy after treatment and changes in pulmonary oxygenation, the days with of oxygenation,the volume fraction of inspired oxygen, hospitalized days were observed.Result：The effective rate of the experimental group（91.8%） was higher than that of the control group（77.6%）（P〈0.05）, the improvement of oxygen index in the treatment group were better than those of the control group（P〈0.05）,days of oxygen, Fi O2, days in hospital of the test group were less than those of control group（P〈0.05）.Conclusion：Nasal continuous positive airway pressure therapy in children with severe pneumonia has clinical efficacy,and can significantly improve oxygenation,so it is worthy of promoting.
