摘要
目的探讨心肺运动试验(cardiopulmonary exercise testing,CEPT)对冠状动脉轻度狭窄患者的评估价值。方法35例胸痛患者,按冠状动脉CT结果分为冠状动脉狭窄组(狭窄程度50%-70%)20例和冠状动脉正常组15例。2组患者均行CEPT检查,观察并比较2组无氧阈值、二氧化碳通气当量、摄氧通气当量、氧耗量、二氧化碳排出量、呼吸频率、运动中通气量、心率、肺泡通气量、潮气量、死腔通气量等指标。结果冠状动脉狭窄组无氧阈值((12.10±0.23)mL/(kg·min))低于冠状动脉正常组((14.47±1.28)mL/(kg·min))(P〈0.05),二氧化碳排出量((1.07±0.21)L/min)高于冠状动脉正常组((0.81±0.04)L/min)(P〈0.01)。结论 CEPT可通过检测无氧阈值和二氧化碳排出量来判断冠状动脉狭窄对心肌的影响。
Objective To explore the value of cardiopulmonary exercise test(CEPT)to evaluate the severity of mild coronary artery stenosis.Methods According to the coronary CT results,35 patients were divided into coronary stenosis group(n=20)(stenosis:50%to 70%)and normal coronary artery group(n=15).All patients received CEPT to observe the value of anaerobic threshold,ventilatory equivalent for carbon dioxide,oxygen ventilation equivalent,maximal oxygen consumption,carbon dioxide output,breathing rate,ventilatory capacity,heart rate,alveolar ventilation,tidal volume and dead space ventilation.Results Anaerobic threshold was significantly lower in coronary stenosis group((12.10±0.23)mL/(kg·min))than that in normal coronary artery group((14.47±1.28)mL/(kg·min))(P〈0.05),and carbon dioxide output was higher in coronary stenosis group((1.07±0.21)L/min)than that in normal coronary artery group((0.81±0.04)L/min)(P〈0.01).Conclusion CEPT can be used to evaluate the influence of coronary stenosis on myocardium by detecting anaerobic threshold and carbon dioxide output.
