电针加颈部康复操治疗颈源性头痛的临床观察与对焦虑的相关性研究

Clinical Observation on Electro-Acupuncture Correlated with Neck Rehabilitation Exercise in Treatment of Cervical Headache and Preliminary Study on Anxiety

目的:观察电针配合颈部康复操治疗颈源性头痛(cervicogenic headache,CEH)患者的临床疗效。方法:选取符合纳入标准的60例CEH患者随机分成治疗组(电针+康复操组)及对照组(电针组),治疗组采用电针治疗配合自主颈部康复操训练,对照组单纯采用电针治疗。两组患者分别于治疗前及治疗1疗程后,利用视觉模拟评分法(visual analogue scale,VAS)评估头痛程度,评价颈椎关节活动度改善情况,分别进行组内及组间对比;同时利用焦虑自评量表(SAS)评价观察两种治疗方法改善焦虑的情况。结果:两组VAS评分在治疗前后进行组内比较,治疗后明显低于治疗前(P<0.01);治疗前后VAS差值两组间比较,治疗组较对照组下降更为明显(P<0.05)。两组间疗效比较,治疗组的总有效率为96.3%,对照组的总有效率为80.0%(P<0.05);两组治疗后SAS标准分均低于治疗前,组内比较具有统计学差异(P<0.01);治疗后的SAS标准分组间比较治疗组优于对照组。结论:两种方法均可改善CEH患者的头痛程度、颈椎关节活动度,缓解其焦虑症状,电针配合颈部康复操治疗CEH临床效果优于单纯针治疗。

Objective ： To observe the clinical efficacy of electro - acupuncture and neck rehabilitation treatment of cer-vical headache （CEH）. Methods ：60 patients who met the inclusion criteria of CEH were randomly divided into treatmentgroup （EA ＋ rehabilitation exercise group） and control group （EA group）. The treatment group received acupuncturetreatment with self - neck rehabilitation exercise training and the control group only used electro - acupuncture treatment.Two groups were assessed degree of headache with visual analogue scales （ visual analogue scale, VAS） before and aftertreatment. And we evaluated cervical range of motion within the group and between groups while advantages of anxietyrating scale （SAS） were observed between two groups to evaluate the anxiety situation. Results： VAS scores in bothgroups after treatment were significantly lower than that before treatment （ P 〈 0.01 ）. VAS difference in treatment groupdecreased more significantly than that in the control group after treatment （ P 〈 0.05 ）. The treatment group total effec-tive rate was 96.3% and in the control group,the total effective rate was 80.0% （P 〈 0.05）. After treatment, the SASstandard scores were lower than that before treatment and the comparisons were statistically different （ P 〈 0.01 ）. SASstandard in treatment group was better than that in the control group. Conclusion：Both methods can improve the patients＂degree of CEH and cervical range of motion and relieve their anxiety symptoms. The neck rehabilitation operation with e-lectro -acupuncture treatment is superior to simple CEH acupuncture.
