摘要
目的利用实时三维斑点追踪技术定量评估尿毒症不同透析龄患者的左心功能情况,探讨其临床应用价值.方法:随机采集确诊尿毒症维持性血液透析患者,共95 例,收集记录患者的一般资料,95 例患者按照透析龄不同分为三组:透析龄〉6 月-≤12 月30 例(HDA 组),透析龄〉12-≤36 月33 例(HDB 组),透析龄〉36月-≤60 月32 例(HDC 组);另外随机采集对照组正常健康人30 例(N 组).对比各项指标评价结果.结果:病例组与正常对照组比较LV-EDV、LV-DSV 升高、LV-EF降低(P〈0.05).病例组与正常对照组N 组比较LV-GSLS、LV-CSGS、LV-GSAS、LV-GSRS 均减低(P〈0.05),HDA 组与HDB 组比较LV-GSLS、LV-GSAS、LV-GSRS均减低(P〈0.05),HDA 组与HDC 组比较LV-GSRS 减低(P〈0.05).LV-GSAS 与LV-EF 相关性最好(r=-0.705).结论:实时三维斑点追踪技术可以定量评估尿毒症不同透析龄患者的左室收缩功能.
Objective To quantitatively evaluate the left ventricular function by real-time three-dimension speckle tracking imaging(3D-STI)in patients with uremia of differentdialysis ages,and explore its clinical application value.Methods:Random sampling confirmed uremia patients with maintenance hemodialysis(MHD), a total of 95 cases, then tocollect general information of patients, 95 patients with MHD were divided into three different groups: Dialysis ages 〉6 months - ≤ 12 months in 30 cases (group HDA), dialysis 12-≤ 36 months in 33 cases (group HDB), dialysis ages 〉36 months - ≤ 60 months in 32 cases (HDC group); another random collection of normal control group and 30 cases of healthypeople (group N).. Results: Compare HDA, HDB, HDC with group N, LV-EDV, LV-DSV increased, LV-EF decreased (P〈0.05). Comparing the indicators evaluation results.CompareHDA, HDB, HDC with group N, LV-GSLS, LV-CSGS, LV-GSAS, LV-GSRS were decreased (P〈0.05), HDA group and HDB group compared with LV-GSLS, LV-GSAS, LV-GSRSwere decreased (P〈0.05), compare HAD and HDC, the LV-GSRS decreased(P〈0.05). LV-GSAS and LV-EF are most relevant(r=-0.705).Conclusion: Real-time three-dimensionalspeckle tracking imaging can quantitatively estimate the left ventricular systolic function in patients with uremia of different dialysis ages.
