SIMV+PSV机械通气模式对COPD呼吸衰竭患者的疗效观察 被引量:28

摘要 目的:研究同步间歇指令通气+压力支持通气(SIMV+PSV)模式对慢性阻塞性肺疾病(COPD)合并呼吸衰竭患者的治疗效果。方法:选取80例COPD合并呼吸衰竭患者,随机分为观察组和对照组,每组各40例,对照组以双水平气道正压通气(BiPAP)模式治疗,观察组以SIMV+PSV模式治疗,检测和比较两组患者治疗前与治疗后第7天肺功能、血气分析、氧代谢三组指标。结果:治疗前两组患者肺功能、血气分析、氧代谢指标的差异均无显著性(P>0.05);治疗后第7天,观察组肺功能、血气分析、氧代谢指标的改善幅度均高于对照组(P<0.05)。结论:SIMV+PSV模式治疗COPD合并呼吸衰竭患者,在提升肺通气功能、改善机体供氧、纠正体内酸碱平衡方面的疗效均优于BiPAP模式。 Objective:To study the effect of SIMV+PSV mode in COPD patients complicated with respiratory failure.Methods:Eighty patients with COPD complicated with respiratory failure were selected as the research objects and randomly divided into the observation group and the control group.Forty cases were included into each group.The patients in the control group were treated with BiPAP mechanical ventilation mode,while the patients in the observation group were treated with SIMV+PSV mechanical ventilation mode.The pulmonary function indexes,blood gas analysis indexes,oxygen metabolism indexes of the patients in the two groups before the treatment and 7days after the treatment were detected and compared.Results:Before the treatment,there were no significant differences of all the indexes of the patients between the two groups(P〉0.05);on the 7th day after the treatment,the improving magnitude of the pulmonary function indexes,blood gas analysis indexes and oxygen metabolism indexes of the patients in the observation group were higher than those of the control group(P〈0.05).Conclusion:The application of SIMV+PSV mode mechanical ventilation in the treatment of the patients with COPD complicated with respiratory failure can achieve better effects of strengthening pulmonary ventilationfunction,improving the oxygen supplement for the body,and correcting the acid-base unbalance than those of applying BiPAP mechanical ventilation mode.
