医院门诊药房药品服务部处方点评结果分析

Results and Analysis of Prescription Comments in Drug Service Department of Our Hospital Outpatient Pharmacy

目的对医院门诊药房药品服务部处方进行点评与分析,为临床合理用药提供参考。方法依据处方点评指南,采用回顾性分析方法,对医院门诊药房药品服务部2013年1月至7月的处方进行等距离抽样提取,共抽取1 371张处方,对处方的各项基本指标进行审查并逐一登记,并对数据进行统计分析。结果不合理处方占比呈明显下降趋势,从最初的9.20%下降至1.00%。1 371张处方中,重点对抗菌药物、血液制品、中药注射剂、激素、注射剂处方进行审查。平均用药品种数1.9-2.2种,抗感染药物使用率维持在9.4%-17.1%,注射剂的平均使用率最高为3.8%。不规范处方占不合理处方总数的14.81%,不适宜处方占不合理处方总数的85.91%。结论医院门诊药房药品服务部处方用药情况基本合理,但不合理处方问题应值得重视,药师应积极发挥审核和干预作用,促进临床合理用药。

Objective To conduct the comments and analysis on the prescriptions in the drug service department of our hospital outpatient pharmacy and to evaluate the medication rationality of these prescriptions in order to provide reference for clinical rational drug use. Methods According to the prescription comments guidance and by adopting the retrospective analysis method,1 371 pieces of the prescription from January to July 2013 were extracted by using the equidistance sample extraction. The various basic indexes of prescription were checked and registered one by one. The obtained data were carried out the statistical analysis. Results The proportion of unreasonable prescriptions showed a significant decline trend from January to July,which was decreased from initial 9. 20% to 1. 00%.Among 1 371 pieces of prescription,the prescriptions with antimicrobial agents,blood products,traditional Chinese medicine injection,hormones and injections were checked as emphasis. The average variety number of drugs was 1. 9- 2. 2, the use rate of anti- infective drugs maintained in 9. 40%- 17. 1%,the average use rate of injections was highest at 3. 8%. The non- standardized prescriptions accounted for 14. 81% of total number of unreasonable prescriptions and the inappropriate prescriptions accounted for 85. 91% of the total number of unreasonable prescription. Conclusion The drug use situation in the drug service department of our hospital outpatient pharmacy is basically rational, but there are still some problems which deserve to be paid more attention. Pharmacists should actively play the role of checking and intervention to promote rational drug use in clinic.
