摘要
目的探讨癌抗原125、癌抗原199及骨桥蛋白联合检测对卵巢上皮癌(EOC)的诊断价值。方法选择2010年6月至2012年5月在苏州市立医院住院治疗的EOC患者(EOC组)40例、卵巢上皮良性肿瘤患者(BEOT组)40例及门诊体检的健康女性(对照组)40例。检测EOC组、BEOT组治疗前后和对照组血清癌抗原125、癌抗原199及骨桥蛋白水平。评价血清癌抗原125、癌抗原199和骨桥蛋白单独检测及三者联合检测的方法学指标对EOC的诊断价值。结果 EOC组治疗后与治疗前各指标比较差异有统计学意义(P<0.05);治疗后血清癌抗原125、癌抗原199及骨桥蛋白水平EOC组明显高于对照组、BEOT组,EOCⅢ期、EOCⅣ期均明显高于EOCⅠ期、EOCⅡ期(均P<0.05),而对照组与BEOT组比较差异无统计学意义(P>0.05);血清癌抗原125、癌抗原199及骨桥蛋白三者联合检测的敏感性较单独检测大幅提高(P<0.05);骨桥蛋白+癌抗原125联合检测可提高对浆液性EOC的检出率(85.0%),骨桥蛋白+癌抗原199联合检测可提高对黏液性EOC的检出率(87.5%),三者联合检测对黏液性EOC及浆液性EOC的检出率均高于单独检测及2种方法联合检测(均P<0.05)。结论血清癌抗原125、癌抗原199及骨桥蛋白三者联合检测对EOC具有较高的诊断价值。
Objective To investigate the value of combined detection of cancer antigen 125,cancer antigen 199 and osteopontin in the diagnosis of epithelial ovarian cancer(EOC).MethodsForty patients with EOC(EOC group)and 40 patients with benign epithelial ovarian tumors(BEOT group)who were treated in Suzhou Municipal Hospital between June 2010 and May 2012 were selected in this study.In addition,40 healthy subjects were selected as control group.Serum levels of cancer antigen 125,cancer antigen 199 and osteopontin were measured before and after treatment,and diagnostic values of the three indicators and their combination were evaluated.Results There were significant differences in the three indicators before and after treatment in EOC group(P〈0.05).Compared with control or BEOT group,serum levels of cancer antigen 125,cancer antigen 199 and osteopontin significantly increased in EOC group after treatment(P〈0.05).Compared with stageⅠ orⅡ EOC,serum levels of cancer antigen 125,cancer antigen 199 and osteopontin significantly increased in stage Ⅲ and Ⅳ EOC(P〈0.05).No significant differences in the levels of the three indicators between control group and BEOT group(P〉0.05).Compared with separate detection,the sensitivity was significantly increased by combined detection of the three indicators(P〈0.05).The detection rate of serous EOC was increased to 85.0%by combined osteopontin and cancer antigen 125 measurement,and the detection rate of mucinous EOC was increased to 87.5% by combined osteopontin and cancer antigen 199 measurement.Compared with separate measurement or combined measurement of two indicators,the detection rates of serous and mucinous EOC were significantly improved by combined measurement of three indicators(P〈0.05).Conclusion Combined detection of cancer antigen 125,cancer antigen 199 and osteopontin in serum has higher value in the diagnosis of EOC.
