支气管镜活检小标本检测DNA含量对肺癌的诊断价值

摘要 目的:探讨对纤维支气管镜活检小标本行流式细胞仪DNA含量分析对于肺癌诊断的临床价值。方法:通过筛选2010年4月-2011年4月于本院呼吸内科行纤维支气管镜检患者,以30例肺癌患者及35例肺部良性病变患者为研究对象,采用流式细胞术进行DNA含量分析,对比分析两组间DI、异倍体率、SPF、PI的差异,评价其对于肺癌诊断的临床价值。结果:肺癌患者的DI值、异倍体率、SPF及PI均较肺部良性病变患者显著升高(P<0.05);病理组织学检查、DNA含量分析、细胞学检查对于肺癌检测阳性率分别为80.0%、73.3%、43.3%,DNA含量分析、病理组织学检查对于肺癌的诊断阳性率显著高于细胞学检查(P<0.05),而DNA含量分析与病理组织学检查结果无显著差异(P>0.05);此外,采用两者联合检测可进一步提高检出阳性率,可达90.0%。结论:肺癌患者肿瘤组织内DNA含量显著高于正常组织,采用流式细胞术进行DNA含量分析,对于肺癌诊断有一定的临床价值。 Objective:To investigate the diagnosis value of DNA content of the small specimens from bronchoscopic biopsy on lung cancer.Method:30 patients with lung cancer and 35 patients with benign lung disease,which received fiberoptic bronchoscopy,were selected as the research object in our hospital during April 2010 to April 2011, and then DNA content was detected by using flow cytometry to analyze thedifference of DI, aneuploidy rate,SPF,PI between the two groups and to evaluate its clinical value for the diagnosis of lung cancer. Result:The DI,aneuploidy rate,SPF,PI of lung cancer patients were significantly higher than that in the patients with benign lung lesions(P〈0.05),the positive rate for lung cancer detection using histopathological examination,DNA content analysis,cytology were 80.0%,73.3%,43.3%,respectively.The positive rate by DNA content analysis and histopathological examination were significantly higher than cytology(P〈0.05), while the DNA content analysis and histopathological examination showed no significant difference(P〉0.05).In addition,the positive rate by combined detection can be further increased up to 90.0%.Conclusion:DNA content in the tumor tissue of lung cancer are significantly higher than that in normal tissue, DNA content analysis by flow cytometry has an important clinical value for the diagnosis of lung cancer.
