子宫按摩对产妇产后泌乳和预防子宫产后出血的临床观察

Clinical Observation of Uterine Massage on Postpartum Lactation and Prevention of Postpartum Hemorrhage

目的:探讨子宫按摩对产妇产后泌乳和预防子宫产后出血的影响。方法:收集320例经阴道分娩的足月妊娠初产妇,随机分为干预组和对照组,各160例,对照组采用单纯注射催产素预防产后出血。干预组在对照组基础上加用子宫按摩。比较两组产妇的泌乳时间、量,第三产程时间、第三产程出血量,产后24 h出血总量及产后出血发生率。结果:干预组泌乳时间明显短于对照组(P<0.05),泌乳充足率明显高于对照组(P<0.05)。第三产程时间和出血量、总产程出血量、产后出血发生率明显低于对照组(P<0.05)。结论:在产妇分娩过程中在注射催产素、米索前列醇预防产后出血时,增加子宫按摩,可有效缩短产妇泌乳时间,增加泌乳量,缩短第三产程,降低出血量和产后出血率,是值得临床借鉴的一种好方法。

Objective：To investigate the effects of uterine massage on postpartum lactation and prevention of postpartum hemorrhage.Method：320 cases of vaginal delivery full-term pregnancy primipara were randomly divided into intervention group and control group,there were 160 cases in each group,control group was used a simple injecting Oxytocin to prevent postpartum hemorrhage.Intervention group was used uterine massage on basis of control group.The lactation time and amount,time and hemorrhage volume of third labor stage,24 h postpartum bleeding volume and incidence rate of postpartum hemorrhage were compared between two groups.Result：The lactation time in intervention group was significantly shorter than control group（P〈0.05）,lactation adequacy ratio in intervention group was significantly higher than that in control group（P〈0.05）.The time of third labor stage and bleeding amount,the bleeding amount of total production process,incidence rate of postpartum hemorrhage were significantly lower than those in control group（P〈0.05）.Conclusion：During childbirth process,while injecting Oxytocin,Misoprostol to prevent postpartum hemorrhage,increase uterine massage can effectively reduce maternal lactation time,shorten birth process,shortening third lactation,reduced blood loss and postpartum hemorrhage rate,it is a kind of good method worthy of clinical reference.
