摘要
目的:观察中药热敷散外敷治疗兔骨性关节炎模型的组织病理学变化及关节冲洗液中白介素-β1(I L-β1)与肿瘤坏死因子-α(TNF-α)的变化,探讨热敷散对兔骨性关节炎的疗效及其作用机理。方法:采用兔膝关节腔内注射木瓜蛋白酶的方法,建立兔骨性关节炎模型:将30只日本大耳白兔随机分为空白对照组(A组,n=10)、模型对照组(B组,n=10)、热敷散组(C组,n=10)。造模后1周,在3组动物中各随机抽取1只实验动物,空气栓塞法处死后进行患膝关节病理观察,明确造模是否成功。A组每天热水袋热敷膝关节2次;B组造模成功后每天热水袋热敷膝关节2次;C组造模成功后,每天用热敷散热敷治疗2次。在治疗4周后,空气栓塞法处死动物,抽取膝关节液后,切开兔的膝关节,观察膝关节股骨髁关节面大体病理变化及股骨髁关节软骨病理变化,并检测关节冲洗液中IL-β1与TNF-α含量的变化。结果:1在造模成功,给予3组实验动物相应治疗4周后处死实验动物,抽取膝关节液后,做病理观察见A组动物关节及关节软骨正常;B、C组动物关节肿胀,关节软骨面破坏。2B、C组动物关节冲洗液中IL-β1和TNF-α的含量明显高于A组(P<0.05),同时C组动物关节冲洗液中IL-β1和TNF-α的含量明显低于B组(P<0.05)。结论:热敷散能通过降低关节液中IL-β1和TNF-α的含量进而起到对骨关节炎的治疗作用。
Objective:To explore therapeutic effects of ReFu powder in treating rabbit with osteoarthritis and its mechanism by observing the changes of histomorphology in the therapy and its influences on the changes of inter-leukin-β1 (IL-β1) and tumor necrosis factor-α(TNF-α) in joint fluid. Methods:Rabbit model was established by in-jecting papain into knee joint cavity of the rabbit;thirty rabbits were randomized into blank control group (A group, n=10), model control group (B group, n=10) and ReFu powder group (C group, n=10). In one week after establishing the models, one rabbit was chosen from each group randomly, sacrificed by air embolism method and the pathology of knee joint was observed to ensure that the model was successfully established. After successfully establishing the models, A group and B group received the application of hot-water bag to knee joint externally, twice per day;C group accepted fomentation of ReFu powder, twice each day. After treating for four weeks, the rabbits were sacri-ficed by air embolism method to extract joint fluid, knee joint was cut to observe pathological changes of knee joint femoral condyle articular surface and pathological changes of cartilages, the contents of IL-β1 and TNF-αin the joint fluid were detected. Results:①After successfully establishing the models, the rabbits in three groups were given with corresponding treatment for four weeks and sacrificed to extract joint fluid, pathological observation showed that the joint and joint cartilage in A group were normal;the joints of the rabbits in B and C group were swollen, the cartilage surface of the joint was broken.② B and C group were higher than A group significantly in the contents of IL-β1 and TNF-αof joint fluid (P〈0.05), at the same time, C group was lower than B group in the contents of IL-β1 and TNF-αof joint fluid (P〈0.05). Conclusion:ReFu powder is effective in treating osteoarthritis, which might be through decreasing the contents of IL-β1 and TNF-αin the joint fluid.
