厄贝沙坦联合氢氯噻嗪治疗高血压合并糖尿病的临床疗效研究

Effect of Irbesartan Combined with Hydrochlorothiazide on Hypertension Complicated with Diabetes Mellitus

目的观察厄贝沙坦联合氢氯噻嗪治疗高血压合并糖尿病的临床疗效。方法选取2010年6月—2013年8月我院收治的高血压合并糖尿病患者175例,按照随机数字表法将患者分为观察组(87例)和对照组(88例)。对照组患者在常规治疗的基础上给予厄贝沙坦治疗;观察组患者在对照组基础上给予氢氯噻嗪治疗,疗程均为8周。两组患者均于治疗前、治疗4周及治疗8周后测量并记录血压及心率,观察两组临床疗效及治疗期间不良反应发生情况。结果治疗前两组患者收缩压和舒张压比较,差异无统计学意义(P>0.05);治疗4周及8周后观察组收缩压均低于对照组,治疗8周后观察组舒张压低于对照组(P<0.05)。两组患者治疗前、治疗4周及8周后心率比较,差异均无统计学意义(P>0.05)。观察组患者总有效率为89.7%,高于对照组的72.7%(P<0.05)。观察组患者出现头疼3例、面红2例;对照组患者出现头疼2例、面红2例。结论厄贝沙坦联合氢氯噻嗪治疗高血压合并糖尿病疗效确切,能有效提高临床疗效且安全性较好。

Objective To observe the effect of irbesartan combined with hydrochlorothiazide on hypertension compli -cated with diabetes mellitus.Methods From June 2010 to August 2013, 175 hypertension patients complicated with diabetes mellitus were selected in our hospital , and they were divided into observation group （ n=87 ） and control group （ n=88 ） ac-cording to a random number table.Both groups given routine treatment , and control group given extra irbesartan , while observa-tion group given extra irbesartan combined with hydrochlorothiazide; both groups treated for 8 weeks.Blood pressure and heart rate were compared between the two groups before treatment and 4-week, 8-week after treatment; clinical effect and adverse reactions during treatment were observed .Results There was no significant differences of SBP and SDP between the two groups before treatment （P〉0.05）; SBP of observation group was higher than that of control group 4-week, 8-week after treatment （P〈0.05）, respectively; SDP of observation group was higher than that of control group 8-week after treatment （P〈0.05）.There was no significant differences of heart rate between the two groups before treatment and 4-week, 8-week after treatment （P〉0.05）.The total effective rate of observation group was 89.7%, was higher than that of control group of 72.7% （P〈0.05） .In the observation group , 3 cases occurred headache , 2 cases occurred flushing; in the control group , 2 cases occurred headache , 2 cases occurred flushing .Conclusion Irbesartan combined with hydrochlorothiazide has certain effect on hyperten-sion complicated with diabetes mellitus , which can significantly improve the clinical effectiveness , and is safe .
