新生儿重症监护室真菌感染的现状及耐药情况分析 被引量:6

摘要 目的探讨新生儿重症监护室(NICU)真菌感染及药敏情况,为其治疗提供依据。方法收集2011—2013年3年间NICU送检的所有真菌检查结果为阳性的标本,并检测其药敏情况,并对相关数据进行统计和分析。结果 3年间NICU送检的所有细菌及真菌检查阳性结果的标本共10 427例,其中真菌检查结果阳性共687例(6.59%),2011—2013年分别有177例(5.81%)、195例(5.86%)和315例(7.77%);真菌阳性标本中,呼吸道标本共443例(64.48%),大便标本195例(28.38%),血液标本36例(5.24%),泌尿生殖道标本13例(1.89%)。各类阳性标本的三大主要菌种为白色念珠菌、近平滑念珠菌及克柔念珠菌。不同标本来源分离的菌株分布情况有所不同:呼吸道、血液及大便标本中以白色念珠菌为主,而泌尿生殖道标本中以热带念珠菌为主。3年药敏结果分析显示,伏立康唑敏感率为96.9%,氟康唑95.8%,两性霉素B 100.0%。结论真菌培养阳性构成比有逐年增高的趋势;真菌感染部位以呼吸道为主,其次为肠道,而真菌感染的种类以白色念珠菌为主,其次为近平滑念珠菌;药敏结果大部分真菌对抗真菌药物敏感率大于95.0%,最常用抗真菌药氟康唑敏感率为95.8%。
