胸腔镜心脏术后动-静脉二氧化碳分压差与中心静脉血氧饱和度相关性分析 被引量:1

摘要 目的探讨完全胸腔镜体外循环下心脏手术术后动-静脉二氧化碳分压差(Pcv-aCO2)与中心静脉血氧饱和度(ScvO2)的相关性。方法选择我院心脏外科2013年1月至2014年10月连续收治的50例先天性心脏病或风湿性心脏病患者,在全麻低温体外循环完全胸腔镜下行手术治疗,分别监测患者术后不同时间点(0、6、12、18h)Pcv-aCO2、ScvO2、左室射血分数(EF)变化。结果患者行胸腔镜心脏术后同时间点Pcv-aCO2逐渐降低,ScvO2逐渐升高,EF逐渐增加,(P均<0.05);术后Pcv-aCO2与ScvO2、EF均呈负相关(r分别为-0.602、-0.502;P均<0.05)。结论 Pcv-aCO2和ScvO2之间具有相关性,可用于胸腔镜下体外循环手术后心脏功能的评估。 Objective To investigate the correlation of arteriovenous PCO2 difference and central venous oxygen saturation after thoracoscope heart operation .Methods 50 patients with congenital heart disease or rheumatic heart disease were hospitalized during January 2013 to October 2014 using hypothermic cardiopulmonary bypass completely VATS operation treatment .The PcvaCO2 ,ScvO2 and left ventricular ejection fraction (EF) changes of patients were monitored at different time points (0 ,6 ,12 ,18h) after the operation .Results The patient's Pcv‐aCO2 decreased gradually ,while the ScvO2 and EF increased gradually at the same time point after undergoing tho‐racoscopy cardiac surgery (P均 〈0 .05) .The postoperative Pcv‐aCO2 had negative correlation with ScvO2 、EF( r=-0 .602、-0 .502 ;P均 〈0 .05 ) .Conclusion Pcv‐aCO2 have correlation with ScvO2 ,which can be used for the car‐diac function evaluation of thoracoscopic extracorporeal circulation surgery .
