头针与康复训练同步治疗对脑梗死急性期患者上肢肌张力的影响

Effects of Scalp Acupuncture Combined with Motor Rehabilitation Synchronously on Recovery of Upper Limbs Muscle Tension in Acute Infarction Patients

目的:探讨头针与康复训练同步治疗对脑梗死急性期患者上肢肌张力的影响。方法:84例脑梗死急性期患者随机分为两组,治疗组采用头针与康复训练同步治疗,对照组则头针与康复训练相隔4 h以上。两组治疗前后采用修订的Ashworth痉挛评定量表及Fugl-Meyer运动功能评定量表进行疗效评定。结果:治疗第14 d两组对比,治疗组上肢肌张力处于0级的病人比例(19.1%)明显少于对照组(38.1%),肌张力≥Ⅰ级的病人比例(80.9%)明显高于对照组(61.9%);治疗第28 d两组对比,治疗组上肢肌张力处于0级的病人比例(38.1%)明显高于对照组(23.8%),肌张力≥Ⅰ级的病人比例(61.9%)明显低于对照组(76.2%)。治疗第14 d治疗组上肢运动功能评分与对照组相比无明显差异,治疗第28 d治疗组评分高于对照组。结论:头针与康复训练同步治疗脑梗死急性期患者,早期可显著缩短上肢软瘫期,持续干预则抑制其肌张力进一步增高,提高患者的运动功能。

Objective ： To explore the effects of scalp acupuncture combined with motor rehabilitation synchronously on recovery of upper limbs muscle tension in acute infarction patients. Methods ：84 cases of acute infarction patients were randomly divided into two groups. The treatment group was treated with scalp acupuncture combined with motor rehabilitation synchronously and the control group was treated with the two therapies above. But they were at an interval of at lest four hours. Modified Ashworth was used to assess the muscle tension and Fugl - Meyer assessment was applied to evaluate the motor function. Results ： The patients with Ashworth grade 0 in the treatment group （ 19.1% ） was significantly less than that（38.1% ）in the control group and the patients with Ashworth grade 1 or more in the treatment group （ 80.9% ） was prominently more than that in the control group （61.9%） on the 14 th day after treatment. The patients with Ashworth grade 0 in the treatment group （38.1% ） was significantly more than that（23.8% ） in the control group and the patients with Ashworth grade 1 or more in the treatment group（61.9% ） was prominently less than that in the control group（76.2% ） on the 28th day after treatment. The motor function of the two groups showed no difference on the 14 th day after treatment and the assessment of the treatment group was superior to the control group on the 28 th day after treatment. Conclusion：The early training of scalp acupuncture combined with motor rehabilitation synchronously can shorten the period of upper limbs flaccid paralysis. Continuous intervention can inhibit high muscle tension and improve the motor function of the acute infarction patients.
