急性肝功能衰竭伴泌尿系和肺部感染患者的药学监护

Pharmaceutical Care for Anti-infective Therapy in A Patient with Acute Hepatic Failure Accompanied by Urinary Tract and Lung Infection

目的：通过药师在肝功能衰竭患者伴泌尿系和肺部感染治疗过程中的药学监护,分析药师如何在临床中发挥作用。方法：以1例肝功能衰竭伴泌尿系和肺部感染患者为例,跟踪治疗全过程,并参与为其制订个体化用药方案。结果：通过临床药师参与制订用药方案,加强用药的针对性及用药过程的监护,使患者感染得到有效缓解。结论：临床药师参与肝功能衰竭患者伴泌尿系和肺部感染治疗用药会诊,体现了临床药师以患者为中心的药学服务理念,提高了救治的有效率。

Objective： To analyse pharmaceutical care for anti-infective therapy in a patient with acute hepatic failure accompanied by urinary tract and lung infection by the pharmacists, in order to investigate how the pharmacists work in the clinical practice. Methods： In case of the treatment of a patient with with acute hepatic failure accompanied by urinary tract and lung infection,clinical pharmacists took part in the whole process and formulated the individualized regimen. Results： Through the development of clinical pharmacists participating in the regimen,it strengthened the rational use of drugs and monitored the process of the treatment. Finally, the infection of the patients got effectively relieved. Conclusion： The pharmacists participated in the treatment of a patient with acute hepatic failure accompanied by urinary tract and lung infection,which manifested the pharmaceutical service with the idea of taking the patients as the center, thus improving the treatment efficiency.
