2007—2012年陕西省居民个人现金卫生支出变化情况研究

Study on Change of Residents' Personal Out of Pocket Health Expenditure in Shaanxi

目的:对比分析陕西省新医改实施前后6年内,城镇和农村居民人均现金卫生支出的变化情况。方法:利用2007—2012年陕西省卫生总费用来源法数据,计算人均现金卫生支出、人均现金卫生支出占年收入的比重等指标,通过不同年份间的纵向比较,比较分析人均现金卫生支出的变化情况。结果:陕西省2009年新医改实施当年,人均现金卫生支出明显降低,但新医改施行第二年该指标恢复到新医改前水平,并呈继续增长趋势;且城镇和农村居民之间医疗经济负担存在明显不公平,农村高于城镇。结论:陕西省卫生总费用个人现金卫生支出比例有待改善,新医改的实施力度需要加强,城镇和农村居民的医疗经济负担不公平性需要改善。

Objective：To compare and analyze the change of per capita out of pocket（OOP） payments of urban and rural residents before and after new medical reform.Methods：2007—2012 source method data of Shaanxi total health expenditure was used tocalculate indicators of per capita OOP health payment, ratio of per capita in annual income, compare and analyze the changes of percapita OOP health payment based on vertical comparison of different years.Results：In 2009, the first year of implementing new medical reform, the per capita OOP health payment decreased significantly, but the indicator returned to the former level before new medical reform in the second year and continued the increasing trend after the implementation of new medical reform; meanwhile, the medical economic burden among urban and rural residents were obviously unfair, the medical economic burden of rural areas was higherthan urban areas.Conclusion：The proportion of OOP health expenditure in Shaanxi should be improved, the enforcement of the newmedical reform should be strengthened, the inequality of medicine economic burden between urban and rural residents needed to beimproved.
