DTI在放射冠及基底节区的急性脑梗死患者中的应用价值探讨 被引量:6

摘要 目的旨在探讨DTI(磁共振扩散张量成像)技术对于放射冠及基底节区急性脑梗死患者中的参数变化与患者的发病时间及恢复情况的相关性。方法分析2013年3月—2014年3月在浙江省人民医院神经内科接受治疗的急性期脑梗死患者的临床资料。比较入组患者健侧与患侧肢体参数差异,分析各检查参数与患者发病时间、患侧肢体肌力的恢复状况之间的关系。结果本研究共纳入40例患者,患者健侧肢体的ADC值(t=14.89,P<0.01)、FA值(t=9.982,P<0.01)水平均显著高于患侧肢体;患者的发病时间与ADC值(r=-0.376,P=0.020)、FA值(r=-0.361,P=0.022)及r ADC(r=-0.412,P=0.015)均呈线性负相关;肌力完全恢复、部分恢复、无恢复的3组患者ADC值(F=7.407,P<0.01)、FA值(F=3.874,P=0.030)、r ADC(F=7.187,P<0.01)组间比较均有统计学差异。结论 DTI的相关参数如ADC、FA、r ADC等与放射冠及基底节区的急性脑梗死患者的发病时间、肌力恢复状况有着显著的相关性。 Objective Our retrospective study was aimed to analyze the application value of DTI in the corona and basal ganglia in patients with acute cerebral infarction. Methods Clinical data of patients with acute cerebral infarction re- ceived treatment at our hospital from 2013 to 2014 was retrospectively analyzed. The differences of parameters between the contralateral limb and the limbs of the patients were compared. The inspection parameters and the relationship between the onset time of patients and the recovery of affected limb muscle strength were analyzed. Results A total of 40 patients were retrospective analyzed. The level of ADC( t = 14.89 ,P 〈 0.01 ) and FA ( t = 9. 982,P 〈 0.01 ) in the contralateral limb of the patients were obviously higher than in the limbs of the patients. The onset time of patients and the level of ADC ( r = - 0. 376, P = 0.020), FA ( r = - 0.361, P = 0.022) and rADC ( r = - 0.412, P = 0.015 ) all had the negative linear correlation. The comparison of the level of ADC ( F = 7. 407, P 〈 0.01 ), FA ( F = 3. 874, P = 0. 030 ) and rADC ( F = 7.187, P 〈 0.01 ) in the three groups of patients with muscle strength, complete recovery group, partial recovery, no recov- ery group had significant difference. Conclusion DTI parameters such as ADC, FA, rADC and the onset time, recovery condition of myodynamia of patients with acute cerebral infarction in the corona radiata and the basal ganglia had the re- markable correlation.
