天宁饮对风痰瘀阻型偏头痛患者血清5-HT影响

Intervention of Tianning Decoction on 5-HT Level in Migraine Patients of Wind-Phlegm and Blood Stasis Blocking Type

目的:观察天宁饮治疗风痰瘀阻型偏头痛患者的临床疗效及对血清5-羟色胺(5-HT)的影响,探明天宁饮治疗偏头痛的作用机理。方法:将80例风痰瘀阻型偏头痛患者随机分为治疗组和对照组,每组各40例,治疗组口服天宁饮,对照组口服盐酸氟桂利嗪胶囊。以15 d为1个疗程,连续用药2个疗程。观察两组患者用药前后头痛的改善情况及血清5-HT的变化。同时,对药物疗效及安全性等进行分析判定。结果:(1)两组患者治疗后头痛各项积分与治疗前相比均降低(P<0.01);两组组间各项积分差值比较有显著差异(P<0.05)。(2)两组临床综合疗效比较:治疗组总有效率92.5%,对照组总有效率72.5%,经统计学分析差异有显著性意义(P<0.05)。(3)血清5-HT水平:两组患者血清5-HT治疗后与治疗前相比,含量均升高(P<0.01);治疗前后5-HT差值治疗组与对照组相比有显著差异(P<0.05)。结论:天宁饮可明显减少风痰瘀阻型偏头痛患者头痛发作次数、减轻头痛程度、缩短头痛持续时间、减少头痛伴随症状,其可能通过升高血清5-HT水平达到缓解头痛的目的。

Objective ： To observe the clinical effect of Tianning Decoction in patients with migraine （ wind-phlegm and blood stasis type ） and effect on serum 5-HT and to find out the mechanism of Tianning Decoction in the treatment of migraine. Methods ： 80 patients of migraine （ wind-phlegm and blood stasis type ） were randomly divided into treatment group and control group. The treatment group （ 40 cases ） were treated with Tianning Decoction and the control group （ 40 cases ） were treated with flunarizine hydrochloride capsules. Fifteen days was one period of treatment and there were two successive periods. We observed the improvement of headache in migraineurs and the level change of 5-HT before and after administration. According to above experimental data, the drug efficacy and safety was evaluated. Results ： （ 1 ） In comparison with the pre-treatment, the headache scores of two groups decreased significantly （ P〈0.01 ）. The headache scores difference of the treatment group was significantly lower than those of the control group （ P〈0.05 ）. （ 2 ） For the comparison of comprehensive curative effect, the total effective rates of treatment group and control group were 92.5% and 72.5%. （ 3 ） In comparison with the pre-treatment, the 5-HT level of two groups elevated significantly （ P〈0.01 ）. The 5-HT level difference of the treatment group was significantly higher than that of the control group （ P〈0.05 ）. Conclusion ： Tianning Decoction can obviously decrease headache attacks, alleviate headache severity, shorten the duration of headache and reduce the simultaneous phenomenon in patients with migraine of the wind phlegm blood stasis type. Tianning Decoction can elevate the level of 5-HT to relieve the symptoms of migraine.
