延续性护理发展现况

Development status quo of continuity of care

从延续性护理的意义、适用范围、工作模式、质量控制方面,对延续性护理的国内外研究现况进行分析,提出我国应建立完善的延续性护理质量监测体系,提高现有延续性护理团队的服务能力,取得我国医疗卫生政策和财政上的支持。

From the aspects of meaning,scope of application,working mode and quality control of continuity of care,it analyzed the research status quo of continuity of care at home and abroad.It proposed that the perfect quality monitoring system of continuity of care should be established in China,and then to improve the service capacity of existing continuity of care team and attain China’s health policy and financial support.
