摘要
目的 分析老年血液透析患者中心静脉导管相关性感染的发生率、危险因素、病原学,探讨护理干预措施及对策.方法 回顾性分析山东大学齐鲁医院桓台分院128例老年血液透析患者资料,并作统计学分析.结果 20例患者确诊为导管相关性感染,感染率为15.63%,中心静脉导管留置方法、留置时间、合并糖尿病为感染的危险因素,葡萄球菌是老年血透患者导管相关感染的最常见细菌.结论 老年人中心静脉导管存在较高的感染率,严格遵守操作规程,缩短留置时间,及早发现感染迹象并及时给予有效的应对措施,做好导管的维护和居家护理宣教,可有效减少导管相关性感染的发生,延长导管使用寿命.
Objective Cather-related infection is a frequent complication for patients who use cather as dialysis access. The aim of this study was to identify incidence, risk factors, bacterial spectrum of central venous catheter related infections (CRI) in eldly maintenant hemodialysis patients with temporary hemodialysis catheters and to explore the anti-infections treatment and prevention measures. Methods We enrolled 128 eldly maintained-hemodialysis patients with temporary hemodialysis catheters. Risk factors for CRI were explored. Results A total of 20 CRI occurred with an infector rate of 15.63 %. Diabetes mellitus, indwelling ways and time were risk factors of CRI. The most common pathogens were staphylococcus. Conclusions Central venous catheter has high infection rate in eldly hemodialysis patients. Keeping the rules of sterile operation, shortening indwelling time, improving the quality continuously and strengthening the pathogen monitoring can reduce the incidence of CRI.
