盐酸美金刚治疗老年帕金森痴呆(PDD)临床疗效及对认知功能的影响

The clinical curative effect of memantine hydrochloride in treatment of senile Parkinson dementia(PDD) and its analysis of influence on cognitive function

目的探讨盐酸美金刚治疗老年帕金森痴呆(PDD)的临床疗效及对认知功能的影响。方法选取我院收治老年帕金森痴呆(PDD)患者64例为研究对象,采用双数字法随机分成A、B两组,每组32例。A组患者口服盐酸美金刚维持治疗,B组则口服盐酸多奈哌齐治疗。记录两组患者Mo CA评分、MMSE评分及ADL评分情况,对比其不良反应发生率。结果 1治疗后,A组Mo CA评分及MMSE评分分别为(23.42±2.82)分和(22.47±3.05)分,明显优于B组的(19.67±2.77)分和(18.95±3.12)分,差异有统计学意义(P<0.05);2A组、B组生活基本完成率及生活完全需要帮助率分别为53.12%、0和28.13%、12.50%,A组明显优于B组,差异有统计学意义(P<0.05);3A组不良反应发生率为15.63%,与B组的25.00%对比无明显差异(P>0.05)。结论盐酸美金刚治疗老年帕金森痴呆效果显著,不良反应少,利于患者认知功能与自理能力提高,可作为临床特效首选药物,有临床推广价值。

Objective To explore the clinical curative effect of memantine hydrochloride in treatment of senile Parkinson dementia （PDD） and its analysis of influence on cognitive function. Methods A total of 64 eases in elderly patients with Parkinson＇s disease dementia（PDD） in our hospital were selected as the research object and were randomly divided into group A and group B using dual digital method, 32 patients in each group. The patients in Group A took 0ral me- mantine hydroehloride to maintain the treatment while those in Group B received the treatment by taking oral donepezil hydrochloride. Recorded of two patients MoCA score, MMSE score and ADL score in two groups were recorded and the incidence of adverse reactions was compared. Re.suits （1） after treatment, MoCA score and MMSE score in A group were respectively （23.42±2.82） and （22.47±3.05）, which were significantly better than those of the B group （19.67± 2.77） and （18.95±3.12）, and the comparison between the two groups had obvious difference （P〈0.05）; （2） Life basically completed rate and life completely need help were respectively 53.12%, 0 and 28.13%, 12.50%. That in group A was better than that in group B. The comparison between the two groups had statistical significance （P〈0.05）; （3） the incidence rate of adverse reaction in group A was 15.63%. Compared with 25.00% of group B, there was no obvious difference（P〉0.05）. Conclusion The treatment of senile Parkinson dementia by using memantine hydrochloride has obvious effect and has less adverse reaction, which can improve the cognitive function of patients with self-care ability and can be used as the preferred clinical drug. This treatment has clinical value.
