小剂量罗红霉素联合不同药物治疗支气管扩张症的疗效观察 被引量:10

摘要 目的:观察评价小剂量罗红霉素联合不同药物在支气管扩张症治疗中的临床疗效。方法:选取2011年1月-2014年1月就诊于本院的86例支气管扩张患者为研究对象,按照随机数字表法分为布地奈德组与氨茶碱组各43例,氨茶碱组应用小剂量罗红霉素+氨茶碱治疗,布地奈德组应用小剂量罗红霉素+布地奈德气雾剂治疗,3个月后对相关指标进行评价。结果:两组治疗前肺功能参数差异无统计学意义(P>0.05),治疗后各组肺功能均有所改善,且布地奈德组肺功能改善程度优于氨茶碱组,差异具有统计学意义(P<0.05)。两组治疗前咳痰量与气促指数组间比较差异无统计学意义,治疗后咳痰及呼吸困难情况均有改善,且布地奈德组的咳痰量与气促指数明显低于氨茶碱组,差异具有统计学意义(P<0.05)。氨茶碱组治疗总有效率为74.4%,布地奈德组总有效率为90.7%,组间比较差异具有统计学意义(P<0.05)。结论:小剂量罗红霉素联合布地奈德气雾剂可显著改善支气管扩张症患者的肺功能及咳痰、呼吸困难等症状,临床效果显著,可作为长期治疗支气管扩张症的有效选择。 Objective:To observe and evaluate clinical effects of low-dose roxithromycin combined with different drugs in the treatment of bronchiectasis.Method:86 cases of patients with bronchiectasis from January 2011 to January 2014 of our hospital were selected as research objects and randomly divided into Budesonide group and Aminophylline group, with 43 cases in each group.Patients in Aminophylline group were treated with low-dose roxithromycin+Aminophylline,and patients in Budesonide group were treated with low-dose roxithromycin + Budesonide Nasal Spray.Related indicators were compared and evaluated after three months' treatment.Result:Before treatment,there was no significant difference in pulmonary function parameters(P〉0.05);after treatment,pulmonary function of patients in each group were improved,with Budesonide group better than Aminophylline group.The difference was statistically significant(P〈0.05).Before treatment,there was no significant difference in sputum volume and dyspnea index;after treatment, sputum and dyspnea were improved in both groups,with sputum volume and dyspnea index in Budesonide group significantly lower that that of Aminophylline group.The difference was statistically significant(P〈0.05).The total effective rate of Aminophylline group was 74.4%,and 90.7% for Budesonide group.The difference was statistically significant(P〈0.05).Conclusion:Low-dose erythromycin combined with Budesonide Nasal Spray inhaler can significantly improve pulmonary function,sputum,and dyspnea symptoms of patients with bronchiectasis.The clinical effect is significant,so that it can be used for long-term treatment of bronchiectasis.
