多排螺旋CT在婴幼儿肺动脉吊带诊断中的应用价值

摘要 目的探讨多排螺旋CT(MSCT)在婴幼儿肺动脉吊带诊断中的应用价值。方法收集2010年12月—2013年12月经手术证实的肺动脉吊带患儿5例,回顾性分析其MSCTA特征。结果 5例肺动脉吊带患儿术前MSCTA均正确诊断,其中合并动脉导管未闭1例,室间隔缺损2例,房间隔缺损1例,主动脉弓上分支异常1例;气道重建显示气管性支气管1例,气管狭窄3例,右侧主支气管延长并气管分叉角度增大5例;合并肺部感染5例。结论 MSCTA不仅能清楚显示婴幼儿肺动脉吊带及其合并心血管畸形,更能明确肺动脉吊带患儿的气管、支气管受压的程度及其狭窄范围,有助于临床明确诊断并选择恰当的治疗方法。 Objective To explore the value of multi slice spiral CT angiography(MSCTA) in diagnosis of pulmonary ar- tery sling(PAS). Methods Collected from December 2010 to December 2013, MSCTA findings of 5 infants with PAS, confirmed by surgical operations were analysed restrospectively. Results All 5 cases of PAS were correctly diagnosed by MSCTA. 1 of the 5 cases was patent duetus arteriosus (PDA), 2 cases were ventrieular septal defect (VSD), and 1 case was atrial septal defect (ASD). The airway reconstruction showed 1 case had tracheal bronchus, 2 cases had traeheostenosis ,4 cases had extend lengthen of right principle bronchus and enlarged angle of the bifurcationg of trachea. 5 cases had lung inflammation. Conclusion MSCTA could clearly identify PAS and associated cardiovascular malformation, Moreover, MSCTA could show the degree of compression and narrow range ot tracheal and bronchus in infants with PAS ,which is helpful in clinical diagnosis and appropriate treatment choose.
