原发性输卵管癌的少见影像表现

Rare Imaging Manifestations of Primary Fallopian Tube Carcinoma

目的分析原发性输卵管癌的CT及MRI少见表现,以提高输卵管癌影像诊断的正确性。方法对经手术及病理证实的6例原发性输卵管癌的CT及MRI征象进行回顾性分析。结果 6例中,5例为单侧,1例为双侧。3例影像表现不典型,其中1例仅表现为宫腔大量积液,2例仅表现为输卵管积水;3例影像表现较典型,为附件区实性或囊实性肿块,实性肿块、囊实性肿块的实性部分CT上呈不均匀或均匀软组织密度,在MRI上呈T1WI稍低、T2WI稍高信号;增强扫描轻中度强化。结论原发性输卵管癌典型表现为附件区实性或囊实性肿块,呈腊肠样、蛇形或梭形。不典型表现为附件区未见明显肿块,而仅仅见到一些间接征象,如宫腔积液伴盆腔内种植转移灶、输卵管积水等。

Objective To analyze rare CT and MRI manifestations of primary fallopian tube carcinoma in order to im- prove the imaging diagnostic accuracy of this disease. Methods The CT and MRI manifestations in 6 eases with pathologically-proved primary fallopian tube carcinoma were retrospectively analyzed ： Results Of the 6 cases, the lesion was unilateral in 5 and was bilateral in one. Atypical imaging manifestations were observed in 3 cases, which included pure massive effusion in uterine cavity （n = 1 ） or only bilateral hydrosalpinx （ n = 2）. The remaining 3 patients has typical imaging manifestations ： solid or cystic mass was seen in adnexa, the solid component of the cystic-solid mass displayed homogeneous or inhomogeneous soft tissue density on CT scans, slightly-lower signal on T1 WI and slightly-higher signal on T2WI, and the lesion showed mild-to-moderate enhancement on contrast-enhanced scanning. Conclusion The typical imaging manifestation of primary fallopian tube carcinoma is a solid or cystic-solid mass situated at adnexal region, being sausage- like, snake-like or spindle in shape. Atypical manifestations include indirect sings such as effusion in uterine cavity associated with pelvic planting metastasis, hydrosalpinx, etc.
